2型糖尿病患者体脂分布特点与颈动脉内膜中层厚度的关系

Relationship between the Characteristics of Lipid Body Distribution and Carotid Artery Intima-media Thickness in Patients with Type 2 Diabetic

目的:探讨2型糖尿病患者体脂分布特点与颈动脉内膜中层厚度(IMT)、血清瘦素水平的关系。方法:选择76例2型糖尿病患者(2型糖尿病组)和34例健康体检者(对照组),测量腰围(WC)、臀围(HC)、体重(W)、全身脂肪含量(BF)、躯干脂肪含量(Tr F)、上肢脂肪含量(UF)、下肢脂肪含量(LF),并计算腰臀比(WHR)、BF%、Tr F/BF。并检测IMT、血清瘦素水平。比较两组患者上述各指标的差异,并分析体脂分布、体脂成分及血清瘦素水平于IMT的关系。结果:2型糖尿病组患者WC、WHR、BMI、BF、Tr F、UF、LF、BF%、Tr F/BF、IMT、血清瘦素水平均明显高于对照组(P<0.05);Pearson相关性分析显示,BF、Tr F、UF、LF、BF%、Tr F/BF与IMT、血清瘦素水平均呈正相关(P<0.05),血清瘦素水平与IMT呈正相关(P<0.05);多元线性回归分析显示,Tr F/BF、瘦素是IMT的独立危险因素。结论:2型糖尿病患者的体脂分布存在异常,IMT、血清瘦素水平明显升高,且两者均与脂肪分布特点密切相关。

Objective：To discuss the relationship between the characteristics of lipid body distribution and carotid artery intima-media thickness in patients with type 2 diabetic.Method：76 patients with type 2 diabetes mellitus（type 2 diabetes group） and 34 healthy subjects（control group） were enrolled in this study.WC,HC,W,BF,Tr F,UF,LF,IMT,serum leptin levels were detected,and WHR,BF%,Tr F/BF were calculated.Differences of the above indexes between the two groups were compared,and the relationship between the characteristics of lipid body distribution and carotid artery intima-media thickness on patients with type 2 diabetic were analyzed.Result：WC,WHR,BMI,BF,Tr F,UF,LF,BF%,Tr F/BF,IMT,serum leptin levels of patients with type 2 diabetes were significantly higher than those of control group（P〈0.05）.Pearson correlation analysis showed that BF,Tr F,UF,LF,BF%,Tr F/BF were positively correlated with IMT,serum leptin levels（P〈0.05）,serum leptin level was positively correlated with IMT（P〈0.05）.Multiple regression analysis showed that Tr F/BF and leptin were the independent risk factors of IMT.Conclusion：The distribution of body fat in patients with type 2 diabetes mellitus are abnormal,IMT and leptin increase significantly,and IMT and leptin are closely related with fat distribution characteristics.
