摘要
目的探讨肺腺癌组织中EGFR19/21基因突变与Ki67,P53表达的相关性及临床意义。方法回顾性分析140例病理确诊为肺腺癌的患者,均经Taqman-ARMS法检测肿瘤组织EGFR19/21基因突变状况,并根据结果进行分组;应用免疫组化SP方法检测两组肿瘤组织中Ki67、P53蛋白的表达情况,对两组检测结果并结合其临床和病理资料应用SPSS 18.0进行统计学分析。结果 140例肺腺癌病例中Ki67蛋白高表达率为(32/140,22.9%);P53蛋白阳性表达率为(63/140,45%);Ki67高表达与肿瘤大小、有否淋巴结转移、分化程度、P-TNM分期有显著相关(P<0.001),与EGFR基因突变状况无关(P=0.227,P>0.05)。P53表达与肿瘤大小、有否淋巴结转移、P-TNM分期有关(P<0.05),与EGFR基因突变状况有关(P=0.041,P<0.05)。EGFR基因突变与P53表达存在正相关性(r=0.173,P<0.05),与Ki67高表达不存在相关性(r=-0.102,P>0.05);P53和Ki67表达间存在正相关性(r=0.166,P<0.05)。结论EGFR基因突变,Ki67、P53蛋白表达在肺腺癌的发生、侵袭、转移起重要作用,联合检测对肺腺癌患者病理诊断、恶性程度判断、个体化治疗选择及预后评估有重要临床意义。
Objective To research the relationship between EGFR 19/21 gene mutation in lung adenocarcinoma patients and lung cancer associated proteins of Ki 67 and P53 expressions and clinical significance .Methods Retrospective analysis of 140 cases of pathological diagnosed with lung adenocarcinoma patients specimens , the EGFR19/21 gene mutation in the tumor tissue used Taqman-ARMS assay, and divided into two groups according to the results .The expression of Ki67 and P53 proteins in tumor specimens of two groups used immunohistochemical SP method , The results of two groups of testing and the clinical and pathological data using SPSS 18.0 to statistical analysis.Results In the 140 cases of lung adenocarcinoma cases , Ki67 protein high expression rate (32/140,22.9%); P53 protein positive expression rate (63/140, 45%); Ki67 expression were significantly related with the tumor size , lymph node metastasis , differentiation , P-TNM stage (P〈0.001), and no correlated with the EGFR mutation status (P=0.227, P〉0.227).P53 expression were related with the tumor size, lymph node metastasis and P-TNM staging ( P 〈0.05), associated with EGFR mutation situation (P=0.041, P〈0.05).EGFR gene mutation and P53 positive correlation (r=0.173, P〈0.05), EGFR gene mutation and Ki67 doesn&#39;t exist correlation (r=0.102, P〉0.102); Positive correlations between P53 and Ki67 expressions (r =0.166, P 〈0.05).Conclusions The EGFR gene mutation, the expression of Ki67 and P53 proteins played an important role in lung adenocarcinoma carcinogenesis and invasion and metastasis , the joint detection of lung adenocarcinoma patients with pathologic diagnosis , judging malignant degree, individualized treatment choice and prognosis assessment has important clinical significance .
