团队护理模式在社区卫生服务中的应用体会

Realization of the application of the nursing team mode in the community health service

社区护理是社区卫生服务的重要组成部分,是集预防、保健、临床护理、康复及健康教育为一体的综合性护理服务,它以便捷、有效、经济、可及的服务方式得到了社区居民的广泛认可。传统的以延续医院护理为主的社区护理模式已不能适应社区不同人群个性化的健康需求。近年来,我中心通过开展团队护理模式、打破护士工作的局限性,充分发挥了社区护士的多重角色功能,不失为一种有效可行的社区护理工作方法。

The community nursing is an important component part of the community health services, and is a comprehensive nursing service which includes prevention, health care, clinical nursing, rehabilitation and health education. Its convenient, effective, economic and accessible service method has been well recognized by the community residents. The traditional community nursing mode which is widely used in the hospital cannot adapt to the personalized health requirement of the different community groups. In recent years, the limitation of the nursing work has been broken in our center and the multiple role function of the community nurses has been fully played through developing a team nursing mode, which is a feasible and effective community nursing working method.
