社区高血压患者服药依从性及影响因素分析

Analysis of the medication compliance and the influence factors of the patients with hypertension in the community

目的:分析社区居民中高血压患者的服药依从性及影响因素。方法:对宝山区庙行社区卫生服务中心门诊的1301例高血压患者进行问卷调查,内容包括高血压基本知识、高血压患者对自我病情的了解、服药依从性等。结果:完全遵医嘱服药患者占46.58%;未遵医嘱服药患者占53.42%,主要为间断服药(34.15%)、停药(10.27%)、曾不按规定时间服药(17.25%)。多因素分析显示诊断时血压级别、患者自己认为高血压对健康的影响程度和服用降压药的降压效果是患者服药依从性差的主要因素。结论:通过强化社区高血压患者的健康教育,可提高患者服药依从性。

Objective： To analyze the medication compliance and the influence factors of the hypertension patients in the community residents. Methods： In the outpatient clinic of Miaohang Community Health Service Center of Baoshan District, 1 301 hypertension patients were investigated with a questionnaire including the basic knowlogy of hypertension, the awareness of their disease, medication compliance, etc. Results： The proportion of the patients who completely followed the doctor＇s advices was 46.58% and the proportion of those who disobeyed the doctor＇s advices was 53.42%, which mainly included the intermittent medication（34.15%）, medication withdrawal（10.27%）, and once not following the prescribed time medication（17.25%）. The multivariate analysis showed that the hypertension degree diagnosed, and the influence degree of hypertension to health that the patients thought and the antihypertensive effect of the antihypertensive drugs taken were the main factors in the patients with the poor medication compliance. Conclusion： Intensification of the health education for the patients with hypertension in the community can improve their medication compliance.
