腰硬联合麻醉在老年患者下肢骨折手术中的应用

The Clinical Effects of Waist Hard Joint Anesthesia on Elderly Patients with Lower Limb Fracture Surgery

目的探讨腰硬联合麻醉在老年患者下肢骨折手术中的临床应用效果。方法筛选2013年1月至2014年9月我院收治的老年下肢骨折患者106例为研究对象,采取随机数表法将其分为对照组与观察组,每组53例,其中对照组行硬膜外阻滞麻醉下进行手术,观察组行腰硬联合麻醉法进行手术,记录两组患者体征参数、出血情况,并统计麻醉起效时间、麻醉满意度及局麻药物辅助剂量。结果观察组术中低血压、高血压发生率明显低于对照组(P<0.05),观察组患者肌肉松弛度明显高于对照组(P<0.05);观察组麻醉起效时间、麻醉满意度及局麻药物辅助剂量明显优于对照组(P<0.05)。结论对于老年患者下肢骨折手术应用腰硬联合麻醉法可获得理想的麻醉效果,并且不良反应发生率较低,麻醉起效较快、麻醉满意度高,适用于手术、麻醉耐受性较差到的老年患者。

Objective To study the clinical effects of waist hard joint anesthesia on elderly patients with lower limb fracture surgery. Methods 106 elderly patients under the lower limb fracture surgery between January and September 2013,who were willing to participate in the test and divided into control group and observation group,53 cases in each group. The control group was treated by epidural block anesthesia surgery,but the observation group was for waist hard joint anesthesia.The signs parameters,bleeding,statistical work time,anesthesia satisfaction and auxiliary dosage anesthesia were recorded. Results The intraoperative blood loss was not significantly different in both groups,but the intraoperative hypotension and hypertension incidence in the observation group was obviously lower than that in the control group. The muscle relaxation degree of the observation group was obviously higher than that of the control group（ P〈0.05）.The anesthesia work time,anesthesia satisfaction and auxiliary dosage anesthesia in the observation group were better than those in the control group（ P〈0.05）.Conclusions The waist hard joint anesthesia which was for the elderly patients with lower limb fracture surgery can obtain the ideal anesthesia effect,a lower incidence of adverse reactions,faster anesthesia working and higher anesthesia satisfaction. It was suitable for elderly patients with poorer anesthesia tolerance.
