乳腺X线可疑恶性钙化对导管原位癌的诊断价值

Diagnostic Value of Breast Mammography Suspicious Malignant Calcification in Ductal Carcinoma in Situ

目的:评价分析乳腺X线可疑恶性钙化在诊断乳腺癌,尤其是导管原位癌中的临床价值,为今后的工作提供参考依据,更好地服务临床。方法:回顾性分析61例乳腺X线检查显示有可疑恶性钙化灶患者的临床资料,所有患者均行术前导丝定位,手术切除定位钙化后进行组织病理学检查,对手术病理诊断为乳腺癌患者的X线钙化特点进行描述和总结。结果:61例患者均成功切除含钙化病变组织,病理结果为导管原位癌13例,浸润性乳腺癌伴原位癌2例,浸润性乳腺癌1例,余良性病变45例。61例患者钙化中,包括点状及圆点状钙化8例,精盐型钙化36例,模糊或不定型钙化11例,线样钙化6例。结论:乳腺X线导丝定位活检对诊断钙化作为唯一阳性征象的导管原位癌敏感性高,是一种安全有效的检查诊断方法,可提高对乳腺原位癌的诊断率。

Objective： The value of breast mammography suspicious malignant calcification in diagnosis of breast cancer, especially ductal carcinoma in situ was analyzed, aiming to provide evidence for clinical practice.Method： The clinical and mammography imaging data of 61 patients with suspicious malignant calcification were retrospectively analyzed.All 61 patients underwent preoperative guide wire position, and pathological diagnosis of excised calcification.The characteristics of calcification of breast cancer patients were described and analyzed.Result ： All calcification in 61patients were successfully removed, 13patients with ductal carcinoma in situ, 2 patients with invasive breast cancer, 1 patient with invasive breast cancer in pathological diagnosis, the remaining 45 patients were benign breast lesion.All 61 patients included 8 cases of dot and dot-like calcification, 36 cases of salt-type calcification, 11 cases of blurred or stereotypes calcification~ 6 cases of line calcification.Conclusion： Preoperative guide wire position by mammography is sensitive for diagnosis of ductal carcinoma in situ with safety.
