磁共振扩散加权成像在早期前列腺癌诊断和鉴别诊断中的价值

The Value of DWI in the Diagnosis and Differential Diagnosis of Early Prostate Cancer

目的:探讨磁共振扩散加权成像(DWI)在早期前列腺癌诊断和鉴别诊断中的价值。方法:选取2013年7月-2014年10月笔者所在医院收治的41例前列腺癌(PCa组)、39例良性前列腺增生(BPH组)患者为研究对象,均行常规T2WI、动态增强T1WI(DCE-MRI)和DWI扫描,PCa组在肿瘤和无肿瘤区设置兴趣区并测量ADC值,BPH组分别在前列腺中央区和外周带测量ADC值。结果:PCa组41例患者中,良恶性前列腺组织的平均ADC值为(0.98±0.24)×10-3mm2/s和(1.35±0.15)×10-3mm2/s;BPH组39例患者中,中央区和外周带的平均ADC值为(1.38±0.23)×10-3mm2/s和(1.44±0.30)×10-3mm2/s。恶性前列腺组织的平均ADC值明显低于PCa组和BPH组中的良性前列腺组织(P<0.05),其他方面的平均ADC值比较差异无统计学意义(P>0.05)。结论:前列腺癌存在特征性DWI表现,与BPH的DWI表现存在较大的差异,因此DWI在前列腺癌的诊断与鉴别诊断中存在较高的临床应用价值。

Objective：To explore the value of magnetic resonance diffusion weighted imaging（DWI） can in the diagnosis and differential diagnosis of early prostate cancer.Method：From July 2013 to October 2014,41 cases of prostate cancer（PCa group） and 39 patients with benign prostatic hyperplasia（BPH group） were selected as the research objects,both routine T2WI,dynamic enhanced T1WI（DCE） MRI and DWI scan can,PCa group in tumor and tumor area set up interest area and measurement of ADC values,BPH group respectively in the central and peripheral zone of prostate measuring ADC values.Result：41 patients in PCa group,the average ADC values for benign and malignant prostatic tissue were （0.98±0.24）×10-3mm2/s and （1.35±0.15）×10-3mm2/s;39 patients in BPH group,the average ADC values for central and peripheral zone （1.38±0.23）×10-3mm2/s and （1.44±0.30）×10-3mm2/s.Significantly lower than the average ADC values of malignant prostate tissue PCa group and benign prostate tissue in BPH group（P〈0.05）,and other aspects of the average ADC values difference was not statistically significant（P〉0.05）.Conclusion：DWI examination can effectively detect prostate,especially on the tip of the central region and the early prostate cancer,obvious effect,is worth of clinical promotion.
