TLR4基因多态性与结核病易感性的meta分析

A meta-analysis of association between TLR4 gene polymorphisms and tuberculosis susceptibility

摘要 目的系统评价Toll样受体4(TLR4)基因多态性与结核病易感性的关系。方法计算机及手工检索自建库至2014年5月发表的关于TLR4基因Asp299Gly和Thr399Ile多态性与结核病易感性的病例一对照研究,提取有关信息,用固定效应模型和随机效应模型进行meta分析,并根据种族进行亚组分析。结果共纳入7篇文献,病例组2234例,对照组1736名。meta分析结果显示:TLR4Asp299Gly多态性与结核病易感性无显著关联性(G vs A:OR=1.04,95%CI:0.79~1.37,P=0.77;GG vs AA:OR=1.56,95%CI:0.76~3.21,P=0.23;GA vsAA:OR=1.01,95%CI:0.84~1.22,P=0.92;GG vs GA+AA:OR=1.55,95%CI:0.75~3.19,P=0.24;GG+GA vs AA:OR=1.03,95%CI:0.86~1.24,P=0.74)。TLR4 Thr399Ile多态性与结核病易感性也无显著联系(T vs C:CR=1.02,95%CI:0.82~1.27,P=0.86;TT vs CC:OR=1.46,95%CI:0.62~3.42,P=0.39;TC vs CC:OR=0.97,95%CI:0.76~1.23,P=0.81;TT vs TC+CC:OR=1.43,95%CI:0.61~3.35,P=0.41;TT+TC vs CC:OR=0.99,95%CI:0.79~1.26,P=0.97)。并且在亚洲人和非亚洲人中均未见显著相关性。结论TLR4基因Asp299Gly和Thr399Ile多态性与结核病易感性无关。 Objective To systematically assess the association between Toll like receptor 4 (TLR4) gene polymorphisms and tuberculosis susceptibility. Methods We systematically searched the electronic Medline,Embase, The Cochrane Library, CNKI, CBMDisc and WanFang databases for research articles about TLR4 gene polymorpbisms and tuberculosis up to May 2014. The data were calculated by either fixed-effect model or random-effect model. Results Seven case-control studies were identified, involving 2 234 patients and 1 736 controls. There was no significant association between TLR4 gene Asp299Gly polymorphisms and tuberculosis in the allele model (G vs A: OR = 1.04,95% CI : 0. 79~1.37, P= 0. 77), the codominant model (GG vs AA: OR=1.56,95 % CI : 0. 76~ 3.21, P = 0.23 ; GAvs AA: OR 1.01,95% CI :0.84~1.22, P =0.92),the dominant model (GG+GA vs AA: OR =1.03,95% CI : 0. 86~1. 24 , P =0.74) ,and the recessive model (GG vs GA+AA: OR =1.55,95% CI :0. 75~3. 19, P = 0.24). Similarly,there was no significant association between TLR4 gene Thr399Ile polymorphisms and tuberculosis in the allele mode (T vs C: OR =1.02,95% CI=0.82~1.27, P =0.86) ,the codominant model (TT vs CC: OR =1.46,95% CI :0.62~3.42, P =0.39;TC vs CC: OR =0.97,95% CI :0.76~1.23, P =0.81),the dominant mode (TT+TC vs CC: OR =0.99,95% CI:0.79~1.26, P =0.97), and the recessive mode (TT vs TC+CC: OR = 1, 43,95% CI :0.61~3.35, P =0.41). Besides,there were still no significant statistical results in Asian or non-Asian population. Conclusions TLR4 gene Asp299Gly and Thr399Ile polymorphisms are not associated with the susceptibility of tuberculosis.
