超声引导介入对囊实性肿物手术感染影响的研究

Effect of ultrasound-guided interventional surgery on surgical infections in patients with various types of cystic and solid masses

目的研究联合超声引导介入对各种囊实性肿物手术感染的影响,以降低感染率。方法选取2012年1月-2013年1月入院诊治的卵巢囊肿患者120例,根据数字随机表法分为试验组60例和对照组60例;试验组应用超声引导介入手术,对照组应用传统的开腹手术,观察两组患者术后治疗效果、手术感染发生率、手术时间、术中出血量、住院时间以及囊肿清除率;术后常规随访一年,观察两组患者卵巢囊肿复发率。结果总有效率试验组为96.67%、对照组为75.00%;手术感染率试验组1.67%、对照组23.33%;试验组手术时间为(27.7±10.5)min、术中出血量为(5.7±2.8)ml、住院时间为(3.5±1.4)d,对照组分别为(36.2±9.3)min、(43.9±6.6)ml、(4.4±1.8)d;试验组单次手术囊肿清除率为95.00%、对照组为71.67%;1年内囊肿复发率试验组为3.33%、对照组为3.33%。结论联合超声引导治疗各种囊实性肿物,可有效降低术后感染发生率,同时缩短手术时间与住院时间、显著减少术中出血量,提高治疗效果与单次手术囊肿清除率。

OBJECTIVE To investigate the effect of combined ultrasound-guided intervention on surgical infections in the patients with various types of cystic and solid masses so as to reduce the infection rate.METHODS A total of120 patients with ovarian cysts who were enrolled in the hospital from Jan 2012 to Jan 2013 were recruited as the study objects and randomly divided into the experimental group with 60 cases and the control group with 60 cases.The experimental group was treated with the ultrasound-guided interventional surgery,while the control group was given the conventional laparotomy.The postoperative curative effect,incidence of surgical infections,operation duration,intraoperative bleeding volume,length of hospital stay,and clearance rate of masses were observed and compared between the two groups,and the recurrence rate of ovarian cysts was observed after a one-year follow-up.RESULTS The total effective rate was 96.67%in the experimental group,75.00%in the control group;the incidence of surgical infections was 1.67%in the experimental group,23.33%in the control group.The operation duration was（27.7±10.5）min in the experimental group,（36.2±9.3）min in the control group;the intraoperative bleeding volume was（5.7±2.8）ml in the experimental group,（43.9±6.6）ml in the control group;the length of hospital stay was（3.5±1.4）days in the experimental group,（4.4±1.8）days in the control group.The clearance rate of masses in single surgery was 95.00%in the experimental group,71.67%in the control group;the recurrence rate of cysts within one year was 3.33%in the experimental group,3.33%in the control group.CONCLUSION As is used for the treatment of various types of cystic and solid masses,the combined ultrasoundguided intervention can effectively reduce the incidence of postoperative infections,shorten the operation duration and the length of hospital stay,significantly reduce the intraoperative bleeding volume,improve the therapeutic effect,and raise the clearance rate of cysts in single surgery.
