3.0T磁共振非对比剂增强触发血管成像对下肢动脉血管病变的诊断价值 被引量:4

摘要 目的探讨3.0T磁共振非对比剂增强触发血管成像(3D-NC-MRA)对下肢动脉血管病变的诊断价值。方法收集2013年3月至2014年3月接受治疗的下肢动脉血管病变患者30例,分别行3D-NC-MRA检查及下肢动脉数字减影血管造影(DSA)检查。结果 3D-NC-MRA检查的295个节段血管的图像质量分级标准显示其优良率为98.33%(295/300)。根据血管狭窄分级标准:3D-NC-MRA检查显示的295个节段血管中,正常173个节段,不同程度狭窄82个节段,完全闭塞40个节段。DSA检查显示,正常185个,不同程度狭窄75个节段,完全闭塞35个节段。两种检查方法具有较高的一致性(Kappa值=0.91)。结论 3D-NC-MRA检查是一种安全有效的检查方法,它无创、无辐射、对比剂用量少、对比剂更为安全。将3D-NC-MRA检查方法应用于诊断下肢动脉缺血性病变可以提高诊断符合率、提高检查的成功率,为临床提供高质量的图像,具有较好的临床应用价值。 Objective To study the diagnostic value of on 3D- NC- MRA enhanced trigger angiography in the diagnosis of lower extremity arterial disease. Methods Lower extremity arteries in patients with vascular lesions of 30 patients from March 2013 to March 2014 were included into this study. 3D- NC- MRA examinations were performed,lower extremity arterial digital subtraction angiography( DSA) examination.The two physicians check the image quality and vascular stenosis. The sensitivity,specificity of the two methods were compared. Results The rate of excellent and good display image quality grading standard of 3D- NC- MRA showed 295 of the segmental vessels was 98. 33%( 295 /300). According to the stenosis vascular standard,3D- NC- MRA showed 295 segments in normal blood vessels; 173 segments,different degrees of stenosis in 82 segments,40 segments of complete occlusion. DSA examination showed 185 normal blood vessels,different degrees of stenosis in 75 segments,35 segments of complete occlusion. Consistency of the two inspection methods was higher. Conclusion 3D- NC- MRA examination is a safe and effective method of examination. It is non- invasive,no radiation,the dosage of contrast medium,contrast agent is more safe and less. The 3D- NC- MRA inspection method is applied in the diagnosis of lower extremity artery ischemic lesions,this method can improve the diagnosis rate,improve the success rate of inspection,provide high quality images for clinical,has better clinical application value.
