玉屏风散联合小剂量低分子肝素对儿童过敏性紫癜患儿氧化应激状态及早期肾损害影响的研究 被引量:16

摘要 目的观察玉屏风散联合小剂量低分子肝素对儿童过敏性紫癜氧化应激状态及早期肾损害的影响。方法将80例过敏性紫癜患儿随机分为观察组(n=40)和对照组(n=40)。对照组给予抗组胺药、糖皮质激素、维生素C及葡萄糖酸钙等常规治疗,观察组在常规治疗的基础上口服玉屏风散5 g/次,3次/d,皮下注射低分子肝素钙10 U/(kg·d),两组治疗疗程均为2周。记录两组患儿治疗前后的超氧化物歧化酶水平(SOD)、血浆丙二醛含量(MDA)、8-异前列腺素F2α(8-iso-PGF2α)、血肌酐及尿素氮、尿微量白蛋白(Alb)及N-乙酰-β-D-氨基葡萄糖苷酶(NAG)含量,并评价两组治疗后的临床疗效并记录两组治疗后发生肾损害及肾炎的情况。结果与对照组比较,治疗组在治疗后14 d的血浆SOD高于对照组(P<0.05),MDA及8-iso-PGF2α低于对照组(P<0.05);两组Alb/Cr及NAG/Cr治疗后2周后都明显降低,但治疗组在治疗7 d、14 d后明显低于对照组(P<0.05);治疗组的治疗后的有效率明显高于对照组(95.5%vs 77.5%;χ2=5.165,P=0.023);治疗组治疗后发生肾损害及肾炎的患者比率明显少于对照组(P<0.05)。结论玉屏风散联合小剂量低分子肝素治疗儿童过敏性紫癜(HSP)疗效显著,能够显著抑制紫癜患儿氧化应激反应,对肾功能具有良好的保护作用。 Objective To observe the Yuping Feng powder combined with small dose low molecular weight heparin on oxidative stress status in children with Henoch Schonlein purpura and Research on early prevention of renal damage. Methods 80 cases of allergic purpura were randomly divided into observation group( n = 40 cases) and control group( n = 40 cases). The control group was given routine treatment antihistamines,corticosteroids,vitamin C and calcium gluconate. The observation group in the conventional treatment based on the oral administration of Yuping Feng powder( 5 g / times,3 times / d),subcutaneous injection of low molecular heparin calcium 10 U /( kg·d). The treatment course of two groups were recorded before and after treatment for 2 weeks. The superoxide dismutase( SOD),plasma malondialdehyde content( MDA),8different prostaglandin F2 alpha( 8- iso- PGF2 alpha),blood creatinine and blood urea nitrogen,urinary albumin( Alb) and N- acetyl beta-D- Glucosaminidase( NAG) content of the two groups were detected,and evaluate the clinical curative effect of two groups after the treatment and the occurrence of renal injury and nephritis recorded in two groups after treatment of the situation. Results Compared with the control group,the treatment group after treatment the plasma SOD of 14 d is higher than that of control group( P〈0. 05). MDA and 8- iso- PGF2 alpha is lower than that of the control group( P〈0. 05). The Alb / Cr and NAG / Cr of two group of after 2 weeks after treatment were significantly lower,but the treatment group in the treatment of 7 d,after 14 d significantly lower than the control group( P〈0. 05). There is significantly higher than that in the control group after treatment in treatment group( 95. 5% vs 77. 5%,χ2= 5. 165,P = 0. 023). The occurrence of renal injury and nephritis treatment group after treatment significantly less than the control group( P〈0. 05). Conclusion Yuping Feng San combined with small dose of low molecular weight heparin in treatment of Henoch Schonlein purpura( HSP) effect is significant. This method can significantly inhibit the oxidative stress reaction in children with Henoch Schonlein purpura,has a good protective effect on renal function.
