低分子肝素钙联合气压波治疗仪预防卵巢癌术后下肢深静脉血栓形成的临床观察

Clinical observation on the application of low molecular heparin calcium combined with Air Pressure Therapy in prevention of deep venous thrombosis

目的观察低分子肝素钙联合气压波治疗仪治疗在预防卵巢癌手术后患者下肢深静脉血栓形成(DVT)的效果。方法 100例卵巢癌手术患者随机分为观察组和对照组,每组50例。两组患者均给予抗感染及营养治疗,观察组加用低分子肝素钙联合气压波治疗仪治疗。分别记录两组患者术前3 d及术后第5天血浆D-二聚体含量、下肢血流平均速度、下肢肿胀、下肢疼痛及下肢深静脉血栓的发生情况。结果观察组患者术后第5天血浆D-二聚体含量较对照组明显减少(P<0.01),下肢血流平均速度较对照组明显增加,下肢肿胀及下肢深静脉血栓发生率较对照组明显降低(P<0.05),而血常规及出凝血时间变化不明显(P>0.05)。结论低分子肝素钙联合空气波压力治疗仪治疗能有效地预防妇科手术后患者下肢DVT的发生,而对凝血机制无影响,安全性好。

Objective To observe the effect of low molecular heparin calcium combined with Air Pressure Therapy on prevention of deep vein thrombosis（DVT） after ovarian cancer surgery. Methods 100 cases of patients undergoing ovarian cancer surgery were randomly divided into Observation Group and Control Group, 50 cases in each group. Two groups of patients were given antibiotics and nutritional treatment, and Observation Group was further treated with low molecular heparin calcium combined with Air Pressure Therapy. D-dimer levels, average blood flow velocity of lower limbs, and occurrence of swelling in lower limbs, leg pain and deep venous thrombosis in lower limbs were recorded 3 days before surgery and 5 days after surgery, respectively. Results On 5th day after operation, plasma D-dimer level of Observation Group was significantly reduced compared with that of Control Group, average blood flow velocity of lower limbs of Observation Group was significantly increased compared with that of Control Group, and the occurrence of swelling and deep venous thrombosis in lower limbs was significantly reduced compared with that of Control Group（P〈0.05）. However, there was no significant change in terms of blood routine and bleeding and clotting time（P〈0.05）. Conclusion Low molecular heparin calcium combined with Air Pressure Therapy can effectively prevent the occurrence of DVT in lower limbs of patients undergoing gynecological surgery, but has no influence on clotting mechanism. It has good safety, and is worth clinical application.
