摘要
目的通过观察2型糖尿病合并OSAHS患者的炎性因子变化情况,以提高对OsAHS的认识并为其治疗提供实验依据。方法选取单纯2型糖尿病患者组(A组)46例、合并OSAHS患者组(B组)61例以及健康体检组(c组)20例。计算各组受试者的体质量指数(bodymassindex,BMI);采用便携式睡眠呼吸检测仪记录患者睡眠情况并计算睡眠呼吸暂停低通气指数( apnea-hypopnea index,AHI) ;应用酶联免疫法测定血清IL-6、肿瘤坏死因子a(tumornecrosisfactor,TNF-α)及可溶性细胞问黏附分子-1(soluble intercellular adhesion molecule-1, sICAM-1)的水平;采用速率散射比浊法测定血清高敏C反应蛋白(high-sensitivity C protein, hs-CRP)含量。结果A组和B组患者各炎症因子水平和BMI均显著高于C组(P值均〈0.01);B组患者各炎症因子水平和BMI均显著高于A组(P〈0.01或P〈0.05)。相关分析显示hs-CRP、sICAM-1及BMI均与并发OSAHS呈独立正相关(P〈0.01或P〈0.05);BMI、TNF-α、sICAM-1和hs—CRP值均与AHI呈独立正相关(P〈0.01或P〈0.05)。结论血清炎症因子水平与糖尿病患者合并OSAHS相关,炎症因子水平增高和BMI是2型糖尿病患者发生OSAHS的危险因素和病情危险因素,应当引起重视。
Objective To analyze the inflammatory factors level in type 2 diabetic patients complicated with obstructive sleep apnea syndrome. Methods A total of 46 type 2 simple diabetes patients were set as group A, 61 type 2 diabetic patients complicated with obstructive sleep apnea syndrome were set as group B,and 20 health controls were set as group C. Body mass index (BMI) was calculated. Apnea-hypopnea index (AHI) was monitored by portable sleep apnea detector. Interleukin-6 (IL-6),tumor necrosis factorα (TNF-α) and soluble intercellular adhesion molecule 1 (sICAM-1) in serum were tested by enzyme-linked immunosorbent assay. High-sensitivity C protein in serum was tested by rate nephelometry. Results All the inflammatory factors and BMI in group A and group B were all significantly higher than those in group C (all P 〈0. 01). There was a significant difference of all the indexes between group A and group B (P 〈0.01 or P〈0.05). BMI, hsCRP and sICAM-1 were independently and positively correlated with patientsr OSAHS risk ( P 〈 0.01 or P 〈 0.05). BMI, TNF-α, hsCRP and sICAM-1 were independently and positively correlated with patientsr AHI. Conclusions The inflammatory factors in type 2 diabetic patients are positively correlated with OSAHS risk and AHI,which makes them worth to be paid attention to.
