垂体后叶素止血药在腹腔镜子宫肌瘤剔除术中的价值研究

Value research of pituitrin hemostatics in laparoscopic hysteromyomectomy

目的 探讨垂体后叶素止血药在腹腔镜子宫肌瘤剔除术中的应用价值,为腹腔镜子宫肌瘤剔除术提供理论依据。方法 选取2012年4月-2013年4月本院收治的70例行腹腔镜子宫肌瘤剔除术的子宫肌瘤患者作为研究对象,将其随机分为对照组和观察组,各35例,对照组给予缩宫素进行治疗,观察组给予垂体后叶素进行治疗,观察比较两组的治疗效果。结果 对照组无效6例,治愈29例,治愈率为82.9%,观察组无效5例,治愈30例,治愈率为85.7%,对照组和观察组的治愈率比较,差异无统计学意义（P〉0.05）;观察组手术时间短于对照组,术中出血量少于对照组,术后血红蛋白下降幅度小于对照组,差异有统计学意义（P〈0.05）;观察组术后排气时间与对照组比较,差异无统计学意义（P〉0.05）;用药15 min后,观察组的收缩压较对照组明显上升,差异有统计学意义（P〈0.05）;用药30 min后,两组患者的舒张压都趋于稳定。结论 垂体后叶素能明显缩短腹腔镜子宫肌瘤剔除术的手术时间,减少术中出血量,提高患者的治愈率,值得在腹腔镜子宫肌瘤剔除术中应用。

Objective To evaluate the application value of pituitrin hemostatics in laparoscopic hysteromyomectomy in order to provide theoretical reference for laparoscopic hysteromyomectomy. Methods Seventy patients with hysteromyoma who received laparoscopic hysteromyomectomy in our hospital from April 2012 to April 2013 were selected and randomly divided into control group and observation group,with 35 patients in each group.Control group was given pitocin treatment and observation group was given pituitrin treatment.The treatment effects between two groups was compared and observed respectively. Results The treatment conditions of control group were as follows：6 patients were ineffective,29 patients were cured,and the cure rate was 82.9%.The treatment conditions of observation group were as follows：5 patients were ineffective,30 patients were cured,and the cure rate was 85.7%.Control group and the treatment group were not statistically different in the cure rate（P〉0.05）.Operative time,intraoperative bleeding amount and postoperative hemoglobin decent degree of observation group reduced significantly compared to that of control group,with statistical difference（P〈0.05）.Control group and treatment group were not statistically different in the anal exhaust time（P〉0.05）.Fifteen minutes after medication,systolic pressure of observation group elevated significantly compared to that of control group,with statistical difference（P〈0.05）.Thirty minutes after medication,diastolic pressure of both groups stabilized. Conclusion Pituitrin can significantly shorten the operative time and intraoperative bleeding amount in laparoscopic hysteromyomectomy and significantly improve the cure rate,thereby worthy of application and recommendation.
