肺癌三维适形放射治疗靶体积确定的影响因素探讨

Influence Factors of Three Dimensional Conformal Radiotherapy for Lung Cancer

目的分析探讨肺癌三维适形放射治疗靶体积的影响因素,为临床合理确定三维适形放射治疗靶体积提供参考依据。方法选取2010年7月～2012年7月来我院就诊治疗的58例肺癌患者的临床资料进行回顾性分析研究,利用TPS观察肺部不同部位的原发病灶随呼吸运动的移动情况,比较不同医师在测量肺癌放射治疗过程中靶区域的不同方向的移动度。结果不同医师对于肺癌放射治疗过程中靶区域的确定存在一定差异性,并且肺癌的运动幅度存在明显个体差异性。结论能否准确确定肿瘤体积（GTV）是影响肺癌三维适形放射治疗靶体积确定的主要影响因素,准确确定GTV是放射治疗成功的关键因素之一,在确定GTV边界值时应该采取个体化放射治疗原则,由于放射治疗医师对于肺癌GTV的认识存在一定差异,因此,在计划靶体积时应该有多位放射医师共同参与并给予确定。

Objective To analyze the factors affecting the target volume of three-dimensional conformal radiotherapy for lung cancer,and to provide a reference for the clinical rational determination of three-dimensional conformal radiation therapy.Methods 58 cases of lung cancer patients with the clinical data were retrospectively analyzed in July 2010～July 2012 in our hospital.The observation of different parts of the lung of the primary lesion with the movement of a respiratory motion,comparing different physicians in the measurement of lung cancer radiotherapy treated region in different directions of mobile degree by TPS.Results Different physicians in the treatment of lung cancer in the process of the target region of the determination of differences,and the movement of lung cancer in the presence of individual differences.Conclusion To determine the tumor volume（GTV） is lung cancer 3-dimensional conformal radiation therapy are the main factors,in determining GTV boundary value take the principle of individualized radiotherapy due to radiation oncologists for lung cancer of GTV awareness there is a difference.Therefore,in the planning target volume have many doctors and reference and determine.
