鼻骨及鼻骨孔的多层螺旋CT分析

摘要 目的探讨鼻骨及鼻骨孔骨折的形态特点及影像学表现,分析多层螺旋CT于鼻骨孔和鼻骨骨折临床诊断中的应用价值。方法选取2012年6月~2014年6月前往我院就诊的70例疑似鼻骨骨折患者及30例健康体检者为研究对象。所有患者均行额骨鼻突至鼻尖CT螺旋扫描,以及高分辨率CT扫描。扫描完成之后进行三维重建和容积再现重建,通过重建图像予以分析鼻骨及鼻骨孔骨折的形态特点及影像学表现。结果 70例疑似鼻骨骨折就诊者中有58例患者确诊为鼻骨骨折,9例未见鼻骨骨折,3例误诊;30例健康体检者CT影像学显示骨孔双侧对称20例,不对称者共计10例,与高分辨率CT扫描结果一致,容积再现重建表现为小孔状影;鼻骨骨折CT横断位表现为纵行线样骨质不连续,冠状位表现为横行或斜行骨质不连续;鼻骨孔CT横断位表现为纵行线样骨质中断影,冠状位表现为横行或斜行骨质不连续,但是骨质光滑自然。结论联合多层螺旋CT高分辨率扫描及三维重建,可良好显示鼻骨及鼻骨孔的解剖结构,对于临床辨别鼻骨骨折及鼻骨孔正常结构具有重要的指导意义。 Objective To investigate the nose and nasal hole fracture morphology characteristics and imaging manifestations and analysis of multi-slice spiral CT in the hole of the nasal bone and nasal bone fracture clinical diagnosis application value.Methods A total of June 2012 to 2014 June went to our hospital 70 cases suspected of nasal bone fracture patients and 30 cases of healthy who as the research object.All patients underwent nasal tip to process CT scan,CT scan and high resolution.Results 70 patients suspected of nasal bone fracture treatment were in 58 patients were diagnosed for nasal bone fracture,9 cases had no nasal bone fracture,3cases were misdiagnosed,30 cases of healthy persons CT imaging showed bilaterally and symmetrically on the bone hole in 20 cases,symmetry in a total of 10 cases,with the high resolution CT scan results consistent,volume rendering reconstruction performances of the keyhole shaped shadow.Conclusion Combined with multi-slice spiral CT high-resolution scanning and three-dimensional reconstruction,good display nasal and nasal foramina anatomical structure and to recognize clinical fracture of nasal bone and nasal bone hole normal structure has important guiding significance.
