舒芬太尼用于危重心脏病患者非体外循环冠状动脉旁路血管移植术麻醉的临床观察

Clinical Observation of Sufentanil in Critically Ill Patients With Heart Disease and Non Anesthesia Off-pump Coronary Artery Bypass Grafting

目的探讨在危重心脏病患者非体外循环冠状动脉旁路血管移植术(OPCABG)麻醉中应用舒芬太尼的临床疗效及安全性。方法选取在我院行OPCABG的58例危重心脏病患者作为研究对象,按照麻醉用药不同分为观察组和对照组,对照组给予芬太尼麻醉,观察组选用舒芬太尼诱导麻醉,术后24 h比较两组手术用时、术前苏醒时间及气管导管拔管时间差异。结果两组左心室射血分数、左室舒张末期内经及手术用时等一般指标对比差异无统计学意义(P>0.05),但观察组术后24 h平均清醒时间和平均拔管时间均短于对照组(P<0.05)。结论对行OPCABG的危重心脏病患者行舒芬太尼诱导麻醉与芬太尼有相似的疗效,但相比之下舒芬太尼麻醉后手术术后清醒时间、气管导管拔管时间更短,可作为快速通道麻醉的理想镇痛药物首选。

Objective To study the patients with severe heart disease of extracorporeal circulation of coronary artery bypass grafting vessels（OPCABG） anesthesia used in the clinical curative effect and safety of sufentanil.Methods 58 patients with OPCABG were selected as the study object in our hospital,according to the drug anesthesia divided into observation group and control group,control group given fentanyl anesthesia,observation group of sufentanil callus induction of anesthesia,postoperative 24 h two groups of operation time,preoperative revival time and trachea extubation time difference.Results Two groups left ventricular ejection fraction,left ventricular end-diastolic canon,and surgery is generally index comparison difference has no statistical significance（P〉0.05）,but the observation group after 24 h average awake time and extubation time were significantly shorter than the control group（P〈0.05）.Conclusion On for OPCABG critical heart disease patients with sufentanil anesthesia induction and fentanyl have similar efficacy,but compared with sufentanil anesthesia postoperative awake time,extubation time shorter tube,as a fast track anesthesia ideal analgesic drug of choice.
