三阴性乳腺癌的临床病理学特征及预后分析

Clinical pathological features and prognosis of patients with triple negative breast cancer

目的探讨三阴性乳腺癌的临床病理学特征及预后特点。方法选择2010年1月至2012年12月西平县人民医院收治的乳腺癌患者90例，将HER-2、PR、ER均为阴性的52例分为三阴组，其他38例分为非三阴组，对比分析两组病理学特征、复发、生存状况等。结果三阴组52例患者中，26例绝经，4例有家族史，47例属于临床Ⅱ、Ⅲ期，8例肿瘤直径在5cm以上，9例组织学分级为Ⅲ级，31例浸润性导管癌，3例浸润性小叶癌，23例病理高度分化，16例有腋窝淋巴结转移。经随访，复发25例，远处转移30例，死亡7例，其中三阴组复发率（36．54％）明显高于非三阴组复发率（15．79％），三阴组远处转移率（42．31％）明显高于非三阴组远处转移率（21．05％），差异有统计学意义（P〈0．05）。结论三阴性乳腺癌是一种独特的亚型，组织分化程度高，多数为浸润性导管癌，其复发率、转移率均较高，整体预后较差。

Objective To investigate clinical pathological features and prognosis of patients with triple negative breast cancer. Methods Ninety patients with breast cancer in the people＇ s hospital of Xiping from January 2010 to De- cember 2012 were selected as study objects, 52 cases of HER-2, PR, ER -negative were divided into triple negative group and other 38 cases were divided into non- triple-negative group. The pathological features, recurrence, survival status of the two groups were analyzed. Results In 52 cases of triple negative group, 26 cases of menopause, 4 cases with family histo- ry, 47 cases at the clinical stage Ⅱ , Ⅲ, 8 cases of cancer than more 5 cm in diameter, 9 cases of histological grade Ⅲ, 31 cases of invasive ductal carcinoma, 3 cases of invasive lobular carcinoma, 23 cases of pathological highly differentiated, 16 cases with axillary lymph node metastasis. After fnllow-up, 25 cases of recurrence, 30 cases of distant metastasis, 7 ca- ses of death, the recurrence rate of triple-negative group（ 36.54% ） was significantly higher than non- triple-negative group （ 15.79% ）, distant metastasis rate of triple-negative group （42. 31% ） was significantly higher than non- triple-negative group （21.05% ） （ P 〈 0. 05 ）. Conclusions Triple negative breast cancer is a unique subtype, a high degree of differen- tiation, the majority of invasive ductal carcinoma, the recurrence rate, higher transfer rates, the overall prognosis is poor.
