特应性皮炎儿童表皮丝聚合蛋白表达的变化 被引量:1

EXPRESSION OF FILAGGRIN IN EPIDERMAL LESIONS OF CHILDREN WITH ATOPIC DERMATITIS

摘要 目的研究特应性皮炎(atopic dermatitis,AD)患儿急性病损期表皮丝聚合蛋白(FLG)的表达情况,从皮肤屏障功能障碍方面探讨特应性皮炎的发病机制,从而为临床治疗提供依据。方法选取中、重度AD患儿作为研究对象(n=15),通过皮肤环钻术获取急性期病损皮肤标本;以正常皮肤为对照(n=15例),采用免疫组化和图像分析系统对丝聚合蛋白的表达进行测定,并对2组儿童表皮丝聚合蛋白的表达情况进行对比分析。结果 AD组患儿表皮丝聚合蛋白染色强度较正常对照组减弱,应用图像分析系统对其染色结果进行分析显示,平均光密度、积分光密度分别为0.208±0.047、4.032±0.919,均明显低于正常对照组(0.296±0.039、9.418±2.264),差异有显著性(P<0.05,P<0.01);阳性面积百分比为(14.6±6.6)%,略低于正常对照组的(16.7±2.4)%,但差异无统计学意义(P>0.05)。结论 AD患儿急性病损期皮肤存在丝聚合蛋白表达的异常,其异常表达导致皮肤屏障功能障碍可能是AD发病的病因之一。 Objective To study the expression of filaggrin (FLG) in acute lesions of atopic dermatitis (AD) children, in order to explore the mechanism of AD and offer the basis to clinical treatment. Methods Skin bi- opsies were collected from acute lesions of AD children (n = 15 ) and normal healthy skin (n= 15 ). The expression of FLG was investigated by immunohistochemistry. Mean optical density (mOD), integral optical density (iOD) and positive area percentage were assessed by image analysis system. Results The staining intensity of FLG protein was decreased in acute lesioned skin of AD subjects. The roOD and iOD in AD group (0. 208±0. 047,4. 032+0. 919 ,respectively) were lower than those in control group (0.296±0. 039,9. 418±2. 264, respectively) ( P〈0. 05, P〈0.01 ). The positive area percentage in AD group[ (14.6+6.6) % ]was slightly lower than in control group[ (16.7±2.4) % 1, but there was no statistically significant difference(P〉0.05 ). Conclusion The expression of filaggrin is abnormal in acute lesioned skin of AD children. Skin barrier dysfunction induced by abnormal expression of filaggrin may be one of the pathogenesis of atopic dermatitis.
