60例宫腔镜下经宫颈子宫纵隔切除术的临床分析

Clinical analysis of 60 cases treated with transcervical resection of septum

目的 分析宫腔镜下经宫颈子宫纵隔切除术（transcervical resection of septum,TCRS）的临床疗效。方法 选取2012年10月至2013年10月首都医科大学附属北京天坛医院首次行TCRS手术治疗60例子宫纵隔患者,术后随访12-24个月,分析患者术后妊娠及分娩结局。结果 不孕率、流产率分别由术前的46.4%、56.7%降至27.5%、5.0%,活产率由术前的6.7%升至53.3%,手术前后比较差异有统计学意义（P〈0.05）,术后均无宫颈机能不全、胎盘早剥、子宫破裂等并发症发生。结论 患者TCRS术后能明显改善妊娠结局。

Objective To summarize and analyze clinical effect of transcervical resection of septum. Methods A total of 60 cases with septate uterus underwent transcervical resection of septum in Beijing Tiantan Hospital Affiliated to Capital Medical University from Oct 2012 to Oct 2013 were selected. All of the patients were followed - up for 12 to 24 months after operation. The pregnant outcomes and delivery outcome were recorded and analyzed. Results Postoperative infertility rate, abortion rate fell to 27.5 %, 56. 7 % from 46.4 %, 5.0 % before surgery, live birth rate rose to 53.3 % from 6.7 %, there were statistically significant differences between postoperative and postoperative （ P 〈 0. 05 ） . No complications such as cervical insufficiency, placental abruption, uterine rupture occurred in the subsequent pregnancy. Conclusion TCRS can obviously improve the pregnancy outcomes .
