脊柱微调手法配合穴位注射治疗椎动脉型颈椎病的临床研究

Clinical Research on Spinal Fine Adjusting Manipulation Combined with Acupoint Injection for Treatment of Vertebral Artery Type of Cervical Spondylosis

目的:探讨脊柱微调手法配合穴位注射治疗椎动脉型颈椎病的临床疗效。方法:将2013年2月—2014年2月收治的椎动脉型颈椎病患者按照数字表法随机分为研究组和对照组,其中研究组患者采用脊柱微调手法配合穴位注射治疗,而对照组则给予西药氟桂利嗪治疗,比较两组患者治疗前后的症状评分、血流动力学情况、颈椎动脉痉挛指数(RI)、寰枢关节的位移指数(ADI)。结果:研究组和对照组患者在治疗后临床症状、体征积分均明显下降,且研究组更显著,差异具有统计学意义(t=5.22,P<0.05)。研究组和对照组在患者在治疗后的VA、BA水平均较治疗前明显升高,但是研究组升高的幅度明显高于对照组,且差异具有统计学意义(t=4.30、3.23,P<0.05)。研究组治疗后颈椎动脉痉挛指数和寰枢关节的位移指数均较治疗前明显下降,对照组治疗后仅颈椎动脉痉挛指数较治疗前明显下降;另外,在治疗后,研究组的颈椎动脉痉挛指数和寰枢关节的位移指数均较对照组明显下降,且差异具有统计学意义(t=3.65、4.12,P<0.05)。结论:脊柱微调手法配合穴位注射治疗椎动脉型颈椎病可明显改善椎动脉型颈椎病患者的眩晕、颈肩痛和头痛等症状,改善颈椎动脉痉挛指数和寰枢关节的位移指数,明显提高椎基底动脉的血流速度,其疗效与脊柱微调手法配合穴位注射治疗改善椎基底动脉供血有关。

Objective： To investigate the clinical efficacy of spinal fine adjusting manipulation combined with acupoint injection for treatment of vertebral artery type of cervical spondylosis. Methods ： In February 2013 - February 2014 the ver- tebral artery type of cervical spondylosis patients according to the random table were randomly divided into study group and control group. The study group patients used spinal fine adjusting manipulation combined with acupoint injection ther- apy, while the control group was given medicine flunarizine treatment. We compared two groups before and after treatment of symptom score, hemodynamics, cervical artery spasm index （RI）, the displacement of atlantoaxial joints index （ADI）. Results： The study group and the control group clinical symptoms and signs scores decreased significantly, and the study group was more obvious,with significant difference （t = 5.22 ,P 〈0.05）. The study group and the control group VA, BA levels were significantly after treatment, but the study group increased more significantly than that in the control group, with statistically significant difference （ = 4.30, t = 4. 30, 3.23, P 〈 0.05 ）. The study group after treatment cervical ar- tery spasm index and the displacement index of atlantoaxial joint were significantly decreased than those before treatment. The control group after treatment only cervical artery spasm index was significantly lower than that before treatment ;in ad- dition, after treatment, cervical artery spasm index and atlantoaxial joint displacement index decreased compared to those of the control group with statistical significance （t = 3.65,4.12 ,P 〈 O. 05 ）. Conclusion： Spinal fine adjusting manipula- tion combined with acupoint injection for treatment of vertebral artery type of cervical spondylosis can significantly improve patients with vertebral artery type cervical spondylosis dizziness, neck pain and headache and other symptoms, improve the index of cervical artery spasm index and the displacement of atlantoaxial joint and obviously improve the blood flow veloci- ty of vertebral artery and basilar artery,improving vertebral basilar artery blood supply.
