颈舒散热奄包外敷联合中医耳穴压豆对甲状腺手术患者VAS评分及PONV症状的影响

Effect of Jingshu Powder Radiating Cover Package External Application Combined with Auricular Plaster Therapy on VAS Score and PONV Symptoms for Thyroid Operation Patients

目的：观察颈舒散热奄包外敷联合中医耳穴压豆对甲状腺手术患者VAS评分及PONV症状的影响。方法：从2013年4月-2014年2月,于我院共有120例病患被诊断需行甲状腺手术。以数字法随机分成观察组（60例）和对照组（60例）。对两组患者均行甲状腺手术。观察组术后给予颈舒散热奄包外敷配合耳穴压豆,对照组术后给予单纯粗盐热奄包外敷加非甾体类解热镇痛药、止吐药等。对比两组患者术后PONV症状的严重程度,以及两种方法的疗效。结果：观察组疗效为优者42例占70.00%,优良率为95.00%（57/60）,显著高于对照组的21.67%（13/60）、50.00%（30/60）,差异均有统计学意义（均P〈0.05）。观察组第1天VAS评分为6.32±1.37分,第2天VAS评分为4.72±0.83分,第3天VAS评分为0.21±1.55分,均显著少于对照组的7.32±2.42分、6.82±1.94分、6.41±1.99分,且观察组患者头痛的VAS评分显著呈下降趋势,差异均有统计学意义（均P〈0.05）。观察组患者术后3天恶心次数为13次,呕吐次数为5次,头晕次数为11次,头痛次数为1次,均显著少于对照组的13次、5次、11次、10次。差异均有统计学意义（均P〈0.05）。结论：颈舒散热奄包外敷颈项部配合王不留行贴压法进行耳穴压豆,痛苦小、副反应少、疗效显著,而且在进行治疗的的过程中,有助于医患之间的沟通,增加了一份人文关怀,具有明显的经济效益及社会效益,值得临床广泛推广。

Objective：To study the effect of Jingshu Powder Radiating Cover Package external application of traditional Chinese medicine combined auricular plaster therapy on patients with VAS score and PONV symptoms of thyroid operation.Methods：From April 2013 to April 2015 in our hospital,a total of 120 cases of patients were diagnosed for thyroid operation.The figures were randomly divided into observation group（60cases）and controlled group（60cases）.On two groups of patients underwent thyroid operation.Jingshu Powder Radiating Cover Package external application combined with auricular plaster therapy for the patients in the observation group,the controlled group was treated with Crude Salt Radiating Cover Package and nonsteroidal anti-inflammatory drugs,antiemetic drugs.Comparison of two groups of patients with the severity of PONV symptoms,and the efficacy of two methods etc..Results：The curative effect of the observation group was excellent accounted for more than70%（42/60）,the excellent and good rate was 95%（57/60）,were significantly higher than those in the controlled group which were 21.67%（13/60）,50%（30/60）.The differences were statistically significant（P〈0.05）.The first day VAS score of the observation group was 6.32±1.37,the second day VAS score was 4.72±0.83,the third day VAS score was 0.21±1.55,significantly less than the controlled group which were 7.32±2.42,6.82±1.94,6.41±1.99.The patients in the observation group were headache and VAS score was significantly decreased.The differences were statistically significant（P〈0.05）.The patients in the observation group after three days of nausea,vomiting,dizziness respectively was 13 times,5times and 11 times,headache for 1times,significantly less than the controlled group of 13 times,5times,11 times,10times.The differences were statistically significant（P〈0.05）.Conclusion：Jingshu Powder Radiating Cover Package externally use at neck and press ear pressure beans with cowherb seed is with small side effect,less pain,a significant effect,but also in the treatment process,contribute to the communication between patient,add a humanistic care,and has obvious economic benefits and social benefits,is worth clinical application.
