不同类型胃转流术治疗2型糖尿病的临床研究

摘要 目的:研究不同类型胃转流术(GBP)对2型糖尿病的治疗效果。方法:将符合条件的2型糖尿病患者随机分为胃空肠襻式吻合转流术组和胃空肠Roux-en-Y吻合转流术组,以血糖和糖化血红蛋白的变化、手术并发症以及手术时间作为观察指标。结果:两组血糖和糖化血红蛋白在术后6个月均下降,且在12个月后保持稳定,胃空肠襻式吻合转流术组和胃空肠Roux-en-Y吻合转流术组的完全缓解率分别为88.7%和86.4%;与胃空肠Roux-en-Y吻合转流术相比,胃空肠襻式吻合转流术具有手术操作方便、并发症少以及手术时间短的优点。结论:胃空肠襻式吻合转流术对2型糖尿病治疗有效,这种术式安全、简便,可以在临床上推广应用。 Objective: To evaluate the effectiveness of different types of gastric bypass(GBP) in treatment of type2 diabetes. Methods: Patients with type 2 diabetes were randomly divided into two groups and treated with gastrojejunal loop anastomosis bypass or gastrojejunal Roux-en-Y anastomosis bypass. Blood glucose and glycated hemoglobin alterations,operation complications and operation time were observed. Results: Blood glucose and glycated hemoglobin were decreased at 6 month and were stable at 12 month after the operation in both groups. The complete response rate was 88. 7% in gastrojejunal loop anastomosis bypass group and 86. 4% in gastrojejunal Roux-en-Y anastomosis bypass group. Gastrojejunal loop anastomosis bypass had the advantages of easier implementation,fewer operation complications and shorter operation time compared with gastrojejunal Roux-en-Y anastomosis bypass. Conclusion: Gastrojejunal loop anastomosis bypass is effective in treatment of type 2 diabetes.It is safe and easier to implement,the method is worthy of clinical popularization.
