脑钠尿肽治疗心肌梗死后肺动脉高压的疗效

摘要 背景人重组脑钠尿肽(rhBNP)是新近应用于临床的强心利钠利尿药物,其对周围血管的影响如对肺动脉压力的影响却尚未有定论。目的对rhBNP治疗肺动脉高压的药效学进行评估,并进一步探讨其机制。方法2012年11月至2013年7月纳入天津市胸科医院心内科急性心肌梗死后发生肺动脉高压的患者60例,随机分为3组,每组20例。分别持续泵入生理盐水(对照组)、前列腺素E1(PGE1)(PGE1组)、rhBNP(rhBNP组)12h。于给药前、给药结束后、给药后1周行心脏超声,检测肺动脉压、左心室舒张末期内径(LVEDD)、左心室射血分数(LVEF);同时检测外周静脉血血栓素A2与环磷酸鸟苷(cGMP)水平。结果 PGE1[(58.6±14.1)降至(33.6±11.8)mm Hg]与rhBNP[(56.8±9.3)降至(43.8±8.1)mm Hg]可有效降低肺动脉压(均P<0.05),但PGE1降肺动脉压同时会伴随体动脉压下降(96.4±9.8降至77.2±11.0mm Hg,P<0.05),rhBNP对体循环无明显影响。三组给药前、给药结束后、给药后1周LVEDD与LVEF差异无统计学意义(均P>0.05)。PGE1[给药结束后(19.5±13.3)比给药前(11.3±8.3)ng/L]与rhBNP[给药结束后(33.8±20.1)比给药前(9.9±8.7)ng/L]均可升高cGMP(均P<0.05),PGE1组血栓素A2水平降低[给药结束后(289±199)比给药前(455±220)ng/L,P<0.05],rhBNP对血栓素A2无明显影响。结论 rhBNP与PGE1用药后均能有效降低肺动脉压,rhBNP的作用更为缓和。
