Research progress on associating liver partition and portal vein ligation for staged hepatectomy

摘要 联合肝实质离断及门静脉结扎的分期肝切除术(ALPPS)是一种新近出现的分期肝切除策略.该法为剩余肝体积(FLR)不足无法一期根治性肝切除手术的原发或转移性肝胆肿瘤患者带来了治愈希望.本文对ALPPS的演进、适应证选择、手术方式、手术安全性及有效性等进行了综述,比较了ALPPS与选择性门静脉栓塞(PVE)两者的效果,并对肿瘤学进展等争议问题进行了探讨. Associating liver partition and portal vein ligation for staged hepatectomy (ALPPS) is a new surgical management which brings hopes of cure to patients with primary or metastatic liver tumor who do not have sufficient future liver remnant (FLR).This review summarizes the current research on the development,indication,surgical procedures and safety of ALPPS.It also discusses the major concerns and unanswered doubts of ALPPS,such as the comparison with selective portal vein embolization (PVE) and the long-term oncological progression.
