浙江省眼科医院2006～2011年斜视手术变化趋势分析

目的探讨斜视住院手术患者斜视类型及年龄构成随年份变化的趋势。方法应用浙江省眼科医院集成化信息系统收集2006～2011年住院斜视患者病历信息，回顾性分析住院斜视患者的性别、年龄及斜视类型的构成。结果2006～2011年住院行斜视矫正术的患者共4460例。共同性外斜视多于共同性内斜视，且有增加趋势，差异有统计学意义（X2=21．212，P〈0．01）。间歇性外斜视是最常见的斜视类型，并有增加趋势，差异有统计学意义（r=17．877，P〈0．01）。6年间，0～6岁，7，17岁和≥18岁三个年龄段的患者比例无明显变化，差异无统计学意义（日：3．667，P〉0．05）。0-6岁组间歇性外斜视患者比例增加，差异有统计学意义（x2=17．184P〈0．01），恒定性外斜视比例减少，差异有统计学意义（r=4．182，P〈O．05）。7—17岁组间歇性外斜视比例增加，差异有统计学意义（矿=13．480，P=0．000），非调节性内斜视减少，差异有统计学意义（，=5．114，F〈O．05）。≥18岁组非调节性内斜视比例减少，差异有统计学意义（r=5．381，P〈0．05）。各年手术患者的性别构成比例无明显差异无统计学意义（r=2．750，P〉0．05）。结论6年中，接受手术治疗的各斜视类型分布发生了变化，外斜视比例增加，内斜视的比例减少。间歇性外斜视是最常见的斜视类型，应成为对未成年斜视患者关注的重点，年轻患者仍是斜视治疗对象中较主要的群体。

Objective To investigate the type and the age structure varies of the strabismus oper- ation inpatients with the year. Methods A retrospective analysis was conducted including the data of all strabismus surgeries performed in Zhejiang Eye hospital from 2006 to 2011, using the integrat- ed information system. The data were analyzed contained the proportions of different surgery types and their association with patients＇ age and gender. Results During the study period, 4460 strabis- mus surgeries were performed. Among all the surgeries, concomitant exotropia was more than comi- rant esotropia, and comitant exotropia increased （X2=21.212, P 〈0.001）. Intermittent exotropia was the most common of all strabismus surgeries and there was an increasing trend （X2=17.877, P 〈0.001）. During the 6 years, the proportions of the three age groups, 0-6 years, 7-17 years, and above 18 years old, had no obvious changes （H=3.667, P 〉0.05）. The proportion of 0-6 years patients with in- termittent exotropia increased （X2=17.184, P 〈0.001）, while constant exotropia decreased （X2=4.182, P 〈 0.05）. The age group of 7-17 years increased in the proportion of patients with intermittent exotro- pia （X2=13.480, P 〈0.001）, while non-accommodative esotropia decreased （X2=5.114, P 〈0.05）. In the age group of above 18 years, non-accommodative esotropia decreased （X2=5.381, P 〈0.05）. The gen- der distribution of the strabismus surgeries had no difference （X2=2.750, P 〉0.05）. Conclusions Dur- ing the 6 years, the distribution of surgery type has changed with the proportion of exotropia increas- ing and esotropia decreasing. Surgery for intermittent exotropia is the most common type of proce- dure, which should become the focus of the juvenile patients with strabismus. Young patients with strabismus are still the major group of the treatment.
