摘要
目的:研究急性胰腺炎(AP)病人的临床流行病学特征。方法:收集1996-2012年本院收治的4 800例AP病人,分析性别、年龄、病因学及预后等临床特征及变化趋势。结果 :男2 832例(59.0%),女1 968例(41.0%),平均年龄分别为(51.5±16.6)岁、(54.1±17.4)岁,发病年龄随时间无明显变化。胆源性(69.0%)、高脂血症性(7.4%)及酒精性(2.3%)是前三位原因。胆源性AP比例稳定(1996-2000年61.9%,2001-2006年70.7%,2007-2012年70.8%);高脂血症性AP比例明显上升AP(1996-2000年2.6%,2001-2006年6.6%,2007-2012年14.1%);酒精性AP比例轻度上升(1996-2000年1.4%,2001-2006年1.7%,2007-2012年3.3%)。各病因之间死亡率无统计学差异(P=0.18)。总体死亡率为4.7%,呈现下降趋势(1996-2000年5.3%,2001-2006年4.8%,2007-2012年4.2%)。平均住院时间为(25.0±34.5)d,住院费用为(75 750±18 000)元(2012可比价格计算),两者均呈下降趋势。结论 :51-60岁为AP最多发年龄段,胆源性是最主要病因,高脂血症性AP增加最显著。
Objective To explore the clinical features and changes in the prevalence of acute pancreatitis(AP).Methods A retrospective study was conducted with 4 800 admissions diagnosed AP in our hospital from 1996 to 2012.The changes in gender, age, etiology, and prognosis were analyzed. Results There were 2 832(59.0%) men and 1 968(41.0%) women with mean age of male(51.5 ±16.6) years and female(54.1±17.4) years without variations in age in this period. Gallstones(69.0%), hyperlipidemia(7.4%) and alcoholism(2.3%) were the top three common causes. The ratio of biliary AP was stable(61.9% from 1996 to 2000, 70.7% from 2001 to 2006 and 70.8% from 2007 to 2012) and that of hyperlipidemic AP increased obviously(2.6%, 6.6% and 14.1% respectively) and alcoholic AP increased slightly(1.4%,1.7% and 3.3% respectively). Etiology-related mortality presented no difference(P =0.18). The mortality was 4.7% and decreased during the period of study from 5.3%(1996-2000) to 4.8%(2001-2006) to 4.2%(2007-2012). The average hospital stay was(25.0±34.5) days and expense(75 750±18 000) RMB, and both decreased in the period of this study.Conclusions The population aging 51-60 was the most vulnerable group for AP all along. Biliary AP was the predominant type of AP and the proportion of hyperlipidemic AP increased obviously.
