多沙唑嗪治疗轻、中度原发性高血压有效性和安全性的Meta分析

Meta-analysis of the Therapeutic Efficacy and Safety of Doxazosin for Mild to Moderate Essential Hypertension

目的系统评价多沙唑嗪治疗轻、中度原发性高血压的有效性和安全性.方法计算机检索Pub Med、CNKI、VIP、CBM、万方等数据库公开发表的以ACEI、ARB、CCB和β-受体阻滞剂为对照的多沙唑嗪治疗轻、中度原发性高血压的随机对照试验,检索年限均为1990年至2014年,提取资料后采用Rev Man 5.2统计学软件进行Meta分析.结果共纳入11篇RCT,9篇为中文,2篇为英文,合计887例患者.Meta分析结果显示,与其他常用抗高血压药物相比,多沙唑嗪治疗轻、中度原发性高血压的临床总有效率[RR=0.95,95%CI(0.89,1.01),P>0.01]、ADR发生率[RR=0.97,95%CI(0.70,1.34),P>0.01]和降低舒张压的能力[SMD=-0.18,95%CI(-0.52,0.16),P>0.01]的差异性均无统计学意义,但降低收缩压的能力有统计学意义[SMD=-0.53,95%CI(-0.90,-0.16),P<0.01].结论多沙唑嗪治疗轻、中度原发性高血压疗效确切,安全性好,尤其适用于伴有良性前列腺增生的高血压患者.

Objective To evaluate the therapeutic efficacy and safety of Doxazosin for mild to moderate essential hypertension. Methods By retrieving from PubMed, CNKI, RCT about the therapeutic Efficacy and Safety of Doxazosin in comparisio VIP, Wangfang database online version, n with ACEI, ARB, CCB and β -receptor blocking treatment of Mild to Moderate Essential Hypertension were collected from 1990 to 2014. Meta-analysis of included trials was performed using Rev Man 5.2 software. Result A total of 11 studies were included, 9 studies were Chinese, and 2 studies were English, involving 887 patients. Meta-analysis showed that there was no statistically significant difference between Doxazosin and others antihypertensive drugs in the therapeutic efficacy [RR=0.95, 95%CI （0.89, 1.01）, P〉0.01], rate ofADR [RR=0.97, 95%CI （0.70, 1.34）, P〉0.01] and Systolic pressure [SMD=-0.18, 95% CI （-0.52, 0.16）, P〉0.01 ]. But there was statistically significant difference in the Diastolic pressure [ SMD =-0.53, 95% CI （-0.90, -0.16）, P 〈0.01]. Conclusion Doxazosin has affirmed effect on mild to moderate essential hypertension, with little side effect, good patient tolerance, especially for hypertension patients with benign prostatic hyperplasia.
