彩色多普勒超声在活体肝移植术中的应用价值（英文）

Application of Color Doppler Ultrasound in Living Donor Liver Transplantation

目的探讨彩色多普勒超声在活体肝移植术中的应用价值.方法选用彩色多普勒手术专用探头,在33例亲体肝移植术中观察记录肝中静脉及其分支结构的解剖信息,监测11例供肝植入血管重建后疑似肝血流异常病例的血管血流通畅情况.结果术中确认37例供体(其中双供体肝移植4例)肝中静脉全程走行并指导供肝切除分界线的选择;手术结束前发现1例肝动脉部分血栓形成、1例肝动脉痉挛、1例门静脉扭曲、1例门静脉急性血栓形成、1例门静脉远端陈旧性机化血栓未彻底取除、1例门静脉吻合口成角狭窄.结论在活体肝移植术中运用彩色多普勒超声可以指导选择最佳的肝脏切面,避免损伤重要结构,准确的评估血管吻合后血流的通畅情况.

Objective To discuss the value of color Doppler ultrasound in the living donor liver transplanta- lion. Methods Designed surgical high frequency probe was selected and used so as to, observe and record dis- section information of middle hepatic vein and branches in 33 cases living related liver transplantation, and monitor 11 cases graft transplant abnormal blood flow after vascular remodeling. Results With the use of color Doppler ultrasound to locate the middle hepatic vein of 37 donors （4 cases of double donor liver transplantation） and clear the cnt boundary of liver in-operation, before the operation, we found 1 case of part hepatic artery thrombus, 1 case of hepatic artery spasm, 1 case of portal vein warped, 1 case of portal vein acute thrombus, 1 case of portal vein ob- solescence organized thrombus not complete remove, 1 case of portal vein anastomotic stoma stricture. Conclusion Color Doppler ultrasound can be used to locate the best cut boundary of liver in living donor liver transplantation, avoid significant structural damaged, exactly evaluate the condition of blood flow.
