硫化氢对急性胰腺炎患者血液流变学的影响

摘要 目的:研究硫化氢(H2S)对急性胰腺炎患者血液流变学的影响。方法:选取重症急性胰腺炎患者(A组)、轻型急性胰腺炎患者(B组)和健康对照者(C组)各40例,留取静脉血测定H2S、血液流变学指标、单核细胞蛋白酶激活受体-1(PAR-1)、组织因子(TF)等含量变化,藉此评价H2S对急性胰腺炎患者血液流变学的影响。结果:A组红细胞聚集指数、血沉、血浆黏度、全血高切黏度、全血低切黏度等均显著高于B组、C组,B组显著高于C组(P<0.01);A组H2S、PAR-1、TF分别为(42.57±7.18)μmol/L、38.53±9.84、(344.24±28.47)ng/L,也分别高于B组的(35.27±3.14)μmol/L、23.42±5.16、(271.67±31.45)ng/L、C组的(24.38±2.69)μmol/L、4.53±2.12、(5.63±3.24)ng/L,B组的这3个指标值分别高于C组的值(P<0.01)。H2S与红细胞聚集指数、血沉、血浆黏度、全血高切黏度、全血低切黏度、PAR-1、TF呈正相关(r分别为0.87、0.74、0.81、0.67、0.78、0.87、0.86,P<0.01)。结论:内源性H2S是一种炎症介质,上调TF、PAR-1表达,引起急性胰腺炎患者血液流变学指标异常。 Objective To study the influence of hydrogen sulfide on haemorheology parameter in patientswith acute pancreatitis.MethodsForty patients suffered from severe acute pancreatitis were selected asgroup A, 40 patients subjected to mild acute pancreatitis were distributed to group B. Healthy persons were included in control group. Blood was used to detect hydrogen sulfide, haemorheology parameter, protease activatedreceptor-l(PAR-1)of monocytes, tissue factor(TF)in order to investigate he influence of hydrogen sulfide on patients' haemorheology parameter with acute pancreatitis..ResultsErythrocyte aggregation index, ESR, plasmaviscosity, high blood viscosity, low blood viscosity in patients with severe acute pancreatitis, was significantlyhigher than that in patients of group B and C. Also, these parameters in group B were predominantly higher thanin group C(P 0.01). H2 S, PAR-1,TF in patients of group A were(42.57±7.18)μmol/L,38.53±9.84,(344.24±28.47)ng/L respectively. In group B, they were(35.27±3.14)μmol/L,23.42±5.16 and(271.67±31.45)ng/L respectively. In group C, these values were(24.38±2.69)μmol/L,4.53±2.12,(5.63±3.24)ng/L. The three valuesin group A were obviously higher than those in group B and C. H2 S positively correlated with erythrocyte aggregation index, ESR, plasma viscosity,high blood viscosity, low blood viscosity, PAR-1, TF(r= 0.87,0.74,0.81,0.67,0.78,0.87,0.86, respectively).Conclusion Endogenous H2 S, as a inflammatory factor, up-regulates PAR-1 and TF which makes haemorheology parameter abnormal in patients with acute pancreatitis.
