摘要
[目的]探讨微生态制剂在根除幽门螺杆菌治疗中的价值。[方法]选取初次就诊仅十二指肠球部活动期溃疡且幽门螺杆菌阳性,并能积极配合完成治疗的患者84例,将其随机分为A组、B组、C组。A组:常规四联疗法(泮托拉唑+胶体果胶铋+阿莫西林+呋喃唑酮),B组:常规四联疗法+双歧三联活菌胶囊,C组:常规四联疗法+乳酸菌素片;3组疗程均为6个月。于治疗后2周、4周,分别检测各组患者的溃疡愈合情况和小肠细菌生长情况,于治疗6个月时检查幽门螺杆菌根除情况,并进行组间两两比较。[结果]1溃疡愈合情况:治疗2周时,愈合总有效率:C组(96.43%)>B组(85.71%)>A组(78.57%),治疗4周时,愈合总有效率:C组(100%)>B组(89.29%)>A组(82.14%),C组与B组、A组比较差异有统计学意义(P<0.05),但B组与A组比较差异无统计学意义(P>0.05)。2小肠细菌生长情况:治疗2周时,3组两两间比较差异有统计学意义(P<0.01);治疗4周时,A组与B组、C组比较差异均有统计学意义(P<0.05),但B组与C组比较差异无统计学意义(P>0.05)。3幽门螺杆菌根除情况:治疗6个月时,幽门螺杆菌根除率C组(92.86%)>B组(82.14%)>A组(57.14%),组间比较差异有统计学意义(P<0.01)。[结论]在根除幽门螺杆菌治疗中,应用微生态制剂能快速、稳定地促进溃疡愈合,并能有效降低肠道菌群失调,显著提高幽门螺杆菌的根除率,应用死菌制剂(乳酸菌素片)效果更明显,值得临床推广。
[Objective]To investigate the clinical efficacy and significance of Helicobacter pylori (Hp) eradicating treatment with oral microbial ecological agent. [Methods]The eighty-four cases with active ulcerative,and Hp positive patients were selected, and randomly divided into three treatment groups (A group, B group, and C group, each group had 28 patients), group A : regular quadruple therapy(Pantoprazole Enteric-Coated Tablets + Colloidal Bismuth Pectin Granules + Amoxicillin Capsules + Furazolidone Tablet) , group B:regular quadruple therapy+Bifid Triple Viable capsule and enteric, group C:regular quadruple therapy+ Lactobacillus tablets. The condition of ulcer healing, intestinal bacteria growth were evaluated at 2 weeks and 4 weeks after the treatment. The HP was detected 6 months after the treatment. [Results] (1) The total efficiency of ulcer healing 2 weeks after the treatment were 96.43% in C group, 85.71% in B group,78.57% in A group. Four weeks after the treatment,it was 100% in C group,89.29% in B group,82.14% in A group. There was no significant difference between A group and B group(P〉 0.05). (2)There was significant difference between each group on the condition of intestinal bacteria growth two weeks after the treatment(P〈0.01). Four weeks after the treatment,there was significant difference between A group and B group,C group. There was no significant difference between B group and C group (P〉0.05). (3)Six months after the treatment,the condition of eradication of Hp was 92. 86% in C group, 82.14% in B group and 57. 14G in A group. There was significant difference between each group(P〈 0.01). [Conclusion]In Hp eradicating treatment,applied micro-ecological agents have better clinical value. It can promote ulcer healing, reduce intestinal flora imbalance, and significantly improve Hp eradication rate, The dead bacterial agents are more effective and worth of clinical use.
