半夏泻心汤对功能性消化不良患者血浆P物质和胃窦黏膜CGRP的影响

Influence of Banxia Xiexin Decoction on Substance P(SP) in the Blood Serum and Calcitonin Gene-related Peptide(CGRP) in the Gastric Antrum Mucosa of Patients with Functional Dyspepsia

目的研究半夏泻心汤对功能性消化不良(FD)患者的临床疗效。通过评价半夏泻心汤对FD患者血浆P物质和胃窦黏膜降钙素基因相关肽(CGRP)的影响,分析其作用机制。方法选取106例FD患者,随机平均分为半夏泻心汤组和吗丁啉组作为对照。以2组患者治疗前后上腹疼痛、上腹饱胀、纳差、嗳气、恶心、呕吐等的症状为观察项目,进行量化评分,评价其综合疗效。应用放射免疫法测定血浆中P物质含量,并用免疫组化技术检测患者胃窦黏膜CGRP的水平。结果给药后,半夏泻心汤组的治疗总有效率为92.45%,显著性高于吗丁啉组(P<0.05),且半夏泻心汤在治疗上腹疼痛、上腹饱胀、纳差、嗳气症状上具有显著性优势(P<0.05);二者对患者血浆P物质含量和胃窦黏膜CGRP表达均显著下降(P<0.05),且半夏泻心汤组下降水平明显高于吗丁啉组(P<0.05)。结论半夏泻心汤可调节FD患者血浆P物质和胃窦黏膜CGRP水平,这可能是其治疗FD的机制之一。

OBJECTIVE To observe the clinic effect of Banxia Xiexin Decoction on patients with functional dyspepsia by e- valuating and analyzing its influence on SP in the blood serum and CGRP in the gastric antrum mucosa as well as the mecha- nism. METHODS 106 cases of patients with functional dyspepsia were selected and randomly divided into the Banxia Xiexin Decoction group and Domperidone group, with 53 patients in each group. Symptoms including the epigastric pain, anorexia, e- ructation, nausea and emesis were observed and the quantitative scores were calculated in both groups before and after the treatment; SP content in the blood serum was tested with radioimmunoassay method, and the CGRP level in the gastric antrum mucosa was detected by immunohistochemistry technique. RESULTS The total effective rate of Banxia Xiexin Decoction group was 92.45 %, which was significantly higher than that of Dornperidone group （P〈0.05）; and Banxia Xiexin Decoction had a clear advantage in improving symptoms such as epigastric pain, anorexia, eructation （P 〈0.05）. The serum SP and CGRP level of both groups remarkably decreased （P〈0.05）, with a more evident level in the Banxia Xiexin Decoction group （P〈 0.05）. CONCLUSION Banxia Xiexin Decoction can effevtively adjust the serum SP and CGRP level, which may be one of the mechanisms of treating functional dyspepsia.
