利用耳甲软骨构建L形支架在鼻综合整形中的应用

Use of L-shaped cartilage bracket building with auricular concha in the nose comprehensive plastic surgery

目的:探讨利用耳甲腔和耳甲艇软骨构建鼻尖L形软骨支架的临床应用。方法:术中完整切取耳甲艇和耳甲腔软骨,根据情况分别用作鼻小柱支撑移植和鼻中隔延伸移植,每部分软骨均从中间剖开,其凸面相对缝合以加强力量并矫直,然后再将两部分软骨缝合后构建牢固的L形软骨支架,使鼻尖抬高和延长。结果:2013年5月-2014年11月,采用该方法进行22例鼻尖重塑手术,矫正短鼻13例,鼻尖突出不足9例。术后随访6-24个月,所有病例鼻尖均有不同程度的延长与抬高(3-8mm),鼻尖形态稳定,较术前有明显改善。结论:单纯采用耳甲艇和耳甲腔软骨构建L形软骨支架的方法能满足轻中度鼻尖延长和抬高的需要,支架牢固,值得临床应用。

Objective To discuss the clinical application of L-shaped cartilage stents which build by auricular concha. Methods Collect cymba conchae and cavum conchae completely,then used as columella support transplantation and extends septum transplantation respectively.Every part of the cartilage sliced convex relative suture to enhance strength and straightening,two parts of cartilage were sutured to build strong L-shaped bracket which can elevate and extend the tip. Results From May 2013 to November 2014,we have finished 22 cases of nasal tip reshaping surgery using this method（13 cases of short nose and 9 cases of lower tip projection）,followed up for 6-24 months after surgery,all the tips have extension and projection of different degree（3～8mm）.Tip shape stability and has obviously improved compared with preoperative. Conclusion Method of rebuilding nasal tip L-shaped cartilage stents using cymba conchae and cavum conchae could meet the needs of mild-to-moderate tip extension and raise,the L-shaped cartilage stent is strong and this method is worthy of clinical application.
