慢性放射性肠炎的研究进展

Research progress in chronic radiation enteritis

慢性放射性肠炎(CRE)是盆腔、腹腔、腹膜后恶性肿瘤经放射治疗引起的肠道并发症之一。CRE主要是由于电离产生的自由基对肠黏膜细胞的损伤导致的,造成的损伤主要包括:肠上皮细胞增生受抑制,肠黏膜下小动脉受损及肠壁组织慢性纤维化;CRE的临床表现为反复发作的便血、腹泻、腹痛,严重者出现肠梗阻、肠穿孔等,外科手术是CRE合并肠梗阻的最终治疗手段,本文就CRE的流行病学,发病机制及临床治疗的最新研究进展予以综述。

Chronic radiation enteritis（ CRE） is a common serious complication in patients receiving abdominal or pelvic irradiation in the course of cancer treatment. CRE is mainly due to free radicals generated by ionization,which causes damage to the intestinal mucosal cells. In this process,intestinal epithelial cell proliferation is inhibited,sub- mucosal arterioles occlusion and intestinal fibrosis is induced. The main clinical manifestations of CRE are blood in stool,diarrhea and abdominal pain. In severe cases,intestinal obstruction and perforation may happen. Surgical treatment is the ultimate treatment for CRE induced- intestinal obstruction. In this paper,we review the epidemiology,pathology and treatment of CRE.
