手术治疗退变性腰椎管狭窄症伴腰椎骨质疏松骨折 被引量:5

Surgical treatment of combined degenerative lumbar spinal stenosis and osteoporotic compression fractures

摘要 目的探讨退变性腰椎管狭窄合并腰椎椎体骨质疏松性骨折的手术治疗及临床疗效。方法对16例退变性腰椎管狭窄合并腰椎椎体骨质疏松性骨折患者采用经肌间隙入路椎板有限切除椎管减压加椎体成形术治疗。结果所有患者手术后随访2年。13例腰痛及下肢放射痛症状消失(其中10例可承担家务劳动),3例明显减轻(其中1例长时间步行或劳动后感下肢麻木)。结论经肌间隙入路椎板有限切除椎管减压加椎体成形术治疗退变性腰椎管狭窄合并腰椎椎体骨质疏松性骨折,创伤小,恢复快,并发症少,是一种较为可靠的手术方式。 Objective To explore the surgical techniques and the clinical outcomes of the treatment of combined degenerative lumbar spinal stenosis and osteoporotic compression fractures. Methods Sixteen patients with degenerative lumbar spinal stenosis combined with osteoporotic compression fractures were treated with limited laminectomy decompressive procedures intramuscularly and vetebroplasty. Results All patients were followed up for 2 years. There were 13 patients with low back pain and sciatica symptoms disappeared( including 10 cases could do housework),3cases were significantly reduced( including 1 case felt numbness after walking or working long time). Conclusions Limited laminectomy decompressive procedures intramuscularly and vertebroplasty are effective,minimally invasive,rapid recovery,lower complications method for patients with degenerative lumbar spinal stenosis combined with osteoporotic vertebral compression fracture.
