经尿道前列腺等离子双极电切术治疗高危良性前列腺增生53例

Clinical effect of transurethral plasmakinetic resection in the treatment of 53 cases of high-risk benign prostatic hyperplasia

目的观察经尿道前列腺等离子双极电切治疗高危良性前列腺增生的临床疗效。方法我院泌尿外科于2010年4月至2012年12月接收高危良性前列腺增生患者106例,随机分为双极电切组(A组)和电切术组(B组),每组53例。A组经尿道前列腺等离子双极电切治疗,B组使用经尿道前列腺电切术治疗,观察两组患者的总有效率,比较分析两组患者的国际前列腺症状评分(IPSS)和生活质量评分(QOL)、最大尿流率和剩余尿量。结果 A组的总有效率为98.11%,高于B组的45.28%,差异有统计学意义(P<0.05);两组患者治疗前后的国际前列腺症状评分、生活质量评分、最大尿流率和剩余尿量比较差异均有统计学意义(P<0.05),A组患者的国际前列腺症状评分、生活质量评分和最大尿流率均明显高于B组,而剩余尿量则明显低于B组,比较差异均具有统计学意义(P<0.05)。结论经尿道前列腺等离子双极电切治疗高危良性前列腺增生不失为一种优良的方法,尤其适用于高龄患者,其不但提高了治愈率,还提高了生活质量,值得临床上推广使用。

Objective To explore the clinical effect of transurethral plasmakinetic resection for treating high-risk benign prostatic hyperplasia. Methods One hundred and six patients with high-risk benign prostatic hyperplasia in the Department of Urology of our hospital from Apr. 2010 to Dec. 2012 were divided into two groups randomly, which were treated with transurethralbipolar plasmakinetic resection（group A, n=53） and transurethral plasmakinetic resection（group B, n=53）, respectively. The total efficiency of the two groups was observed, and the international prostate symptom score（IPSS）, quality of life（QOL）, maximal urinary flow rate and residual urine volume of the two groups were compared and analyzed. Results In group A, the total effective rate was 98.11%, significantly higher than 45.28% in group B（P〈0.05）. There were statistically significant difference before and after treatment in the two groups in IPSS, QOL, maximal urinary flow rate and residual urine volume（P〈0.05）, with IPSS, QOL, maximal urinary flow rate significantly higher in group A than group B and residual urine volume significantly lower in group A（P〈0.05）. Conclusion It can be a good way to treat patients of high-risk benign prostatic hyperplasia with transurethral plasmakinetic resection, especially for elderly patients. It enhances the cure rate and also improves the quality of life, which worth to be widely used in clinical practice.
