急性饮酒早期脑部灌注改变的动脉自旋标记分析 被引量:6

摘要 目的采用三维动脉自旋标记(3D-ASL)技术对急性饮酒后早期脑部灌注(CBF)改变进行量化,探讨酒后CBF改变与脑功能之间的联系。方法将80名志愿者分成试验组和对照组,每组40名(男女各20名),平均年龄(25.2±2.7)岁。志愿者均无个人及家族酗酒史、精神药物以及毒品服用史。两组均常规扫描除外脑部病变,试验组空腹服用一定量白酒,对照组空腹服用等量安慰剂,两组在试验前均进行ASL扫描作为基线,随后试验组饮酒后20 min接受ASL扫描,每次检查前均进行生理指标的监测。利用组间t检验分析所得数据。结果试验组21名出现全脑各叶皮层、丘脑、豆状核、小脑、脑桥的CBF增高(P<0.05),所有脑区左右相比差异均无统计学意义(P>0.05),男性的额叶皮质CBF改变最显著。女性则表现为全脑灰质CBF弥漫性增高,各脑区CBF增加程度差异无统计学意义(P>0.05)。11名枕叶、小脑和脑桥CBF出现下降,差异均有统计学意义(P<0.05)。左右脑区相比未发现有CBF改变差异明显的脑区。丘脑与豆状核CBF改变各不相同,差异均无统计学意义。6名脑部CBF与饮酒前基本相同,差异无统计学意义(P>0.05)。2名因饮酒后感到身体明显不适退出试验。整个试验过程中对照组脑部CBF变化差异均无统计学意义。对照组和试验组饮酒前左、右脑区CBF值差异均无统计学意义。结论饮酒后皮质及灰质核团CBF明显升高55.3%(21/38),28.9%(11/38)下降,余15.8%(6/38)变化不明显。说明饮酒能不同程度改变大脑的CBF,尤其是后循环供应的脑区,从而影响脑功能变化。但变化存在明显个体差异,可能是酒后出现不同情绪和执行力差异的重要因素。 Objective We used 3D-arterial spin labeling technology( 3D-ASL) to measure the early brain perfusion( CBF) quantitative change after acute alcohol,and observed the relationship between the change of CBF after drinking and brain function. Methods 80 volunteers were divided into trial group and the contral group,40 people in each group( 20 men and 20 women),average age 25. 2 ± 2. 7 years old. Volunteers had no personal and family history of alcoholism,mental history of drugs and drug used. Both groups including regular scans brain lesions,the experimental group on an empty stomach amount of liquor,the control group on an empty stomach equivalent placebo,two groups were performed before the test ASL scan as a baseline,after 20 minutes two groups were accepted ASL scanning,the monitoring of physiological indicators before every examination. Using t test analysis data from two groups. Results 21 people in the cerebral cortex,the thalamus and lentiform nucleus,the cerebellum,pontine CBF increased( P〈0. 05) in experienced gruop,all about brain regions compared with no statistical significant difference( P〉0. 05),the CBF changes of the frontal cortical in male is most significant. Women are characterized by whole brain gray matter increased CBF diffuse,the differences in the cerebral regions increase CBF and not statistical significant difference( P〉0. 05). 11 of the occipital lobe,cerebellum,and pons CBF decline,have statistical significance( P〈0. 05). CBF change has not been found between the cerebral regions. The thalamus and lentiform nucleus CBF changes each are not identical. 6 people of brain CBF is same as before,no statistical significance( P〉0. 05). 2 people feel unwell obviously after drinking out of experiment. Brain CBF changes the whole experimental process in the control group had no statistical significance. The control group and experimental had no statistical difference of the value of CBF between two hemispheres. Conclusion This study shows that after drinking alcohol,cortical CBF in grey matter significantly increased 21 /38 people( 55. 3%),11 /38( 28. 9%),more than 6 /38 people does not change significantly( 15. 8%). Drinking can alter brain different level of CBF,especially after the loop supply areas of the brain,which affects brain function change. Obvious individual differences,but the change may be drunk in different mood and execution difference of important factor.
