特发性性早熟患儿垂体MRI表现与黄体生成素及卵泡刺激素激发峰值相关性分析 被引量:2

The Correlation Analysis of MRI Finding of Pituitary Gland Morphology in Idiopathic Central Precocious Puberty with Provocation Peaked of Luteinizing Hormone and Follicle Stimulating Hormone

摘要 目的探讨特发性性早熟(ICPP)患儿的垂体MRI表现及其与临床相关激素的关系,为临床诊断提供影像学依据。方法搜集49例确诊为ICPP的患儿(3.7-8.9岁),男4例,女45例,行垂体MRI检查,观察垂体上缘形态,测量腺垂体各径线并与正常对照组比较,分析其腺垂体各径线及其与卵泡刺激素(FSH)和黄体生成素(LH)激发峰值的关系。结果 49例ICPP患儿中,垂体上缘形态隆凸型45例(91.8%),平坦型4例(8.2%)。男性及女性患儿腺垂体冠状宽径、冠状高径、矢状高径、矢状前后径均比正常对照组增大(P〈0.05),其中冠状高径、矢状高径比正常对照组明显增大(P〈0.01)。ICPP患儿腺垂体冠状高径、矢状高径与FSH和LH激发峰值呈正相关(r=0.43,P〈0.01;r=0.28,P〈0.05)和(r=0.49,P〈0.05;r=0.29,P〈0.05)。结论 ICPP患儿腺垂体体积增大,垂体上缘形态以隆凸型多见,腺垂体冠状高径、矢状高径与FSH、LH激发峰值呈正相关。 Objective To study MRI finding of idiopathic central precocious puberty( ICPP) and the correlation with clinical correlated hormone,and to provide imaging evidence for clinical diagnosis. Methods Collecting 49 patients( 4boys and 45 girls) aged 3. 7- 8. 9 years old with ICPP,pituitary gland were scanned with MRI,observing shape of pituitary gland superior border,measuring each way of adenohypophysis and to compare with normal control,the correlation with each way of adenohypophysis and provocation peak of follicle stimulating hormone( FSH) and luteinizing hormone( LH) was analyzed. Results In 49 patients,prominentia type pituitary gland were 45 cases( 91. 8%) and flat type were 4 cases( 8. 2%). The coronal width diameter and coronal height diameter and sagittal height diameter and sagittal anteroposterior diameter of adenohypophysis were more enlarge obviously than normal control( P 0. 01). The coronal and sagittal height diameter of adenohypophysis with ICPP is positive correlation with provocation peaked of FSH and LH( r = 0. 43,P 0. 01;r = 0. 28,P 0. 05) and( r = 0. 49,P 0. 05; r = 0. 29,P 0. 05). Conclusion The volume of adenohypophysis with ICPP is enlarge,the shape of pituitary gland superior border more displayed with prominentia type. The coronal and sagittal height diameter of adenohypophysis is positive correlation with provocation peaked of FSH and LH.
