两种基于冠状动脉造影图像预测心肌血流储备分数方法的对比研究

摘要 目的比较柏肃叶等式和FAST评分预测心肌血流储备分数(fractional flow reserve,FFR)的差异。方法入选2009年1月1日至2014年12月1日武汉亚洲心脏病医院行冠状动脉造影及FFR测定的冠心病患者80例,同时计算病变长度(lesion length,LL)/[最小管腔直径(minimal lumen diameter,MLD)]4和FAST评分。通过ROC曲线下面积大小,比较两种评估方法预测FFR的准确度。结果柏肃叶等式和FAST评分的ROC曲线下面积分别为(0.674±0.065)和(0.505±0.065),柏肃叶等式的ROC曲线下面积差异具有统计学意义(P<0.05),而FAST评分显示差异无统计学意义(P>0.05)。结论 FAST评分不能预测FFR<0.8,柏肃叶等式的评分能用于预测FFR<0.8,但准确性不高。 Objective To compare the two methods based on either the Poiseuille equation or the FAST (FFR Angiographic Scoring Tool) score in predicting forecasting myocardial fractional flow reserve by coronary coronary angiography images. Methods A total of eighty patients with coronary artery disease undergoing coronary angiography and myocardial fractional flow reserve (FFR) assessment from January 2009 to December 2014 in Wuhan Asia Heart Hospital were enrolled. The LL (lesion lengh) and MLD ( minimal lumen diameter) were recorded followed by calculation of LL/MLD4 and FAST ( FFR Angiographie Scoring Tool) scores. Comparisons of the 2 methods in myocardial fractional flow reserve prediction were carried out using the areas under the ROC ( receiver operating characteristic ) curve. Results The areas under the ROC curve of Poiseuille equation were (0. 674 ±0. 065, P 〈 0. 05 ) , and the FAST scores were (0. 505 ± 0. 065, P 〉 0. 05 ) . Conclusions The FAST score can not predict myocardial ischemia when FFR 〈 0. 8 while poiseuille equation can predict myocardial ischemia for FFR 〈 0. 8 but accuracy is not high.
