胫骨后侧平台骨折采用创伤关节镜下手术治疗的临床研究

Analysis on clinical efficacy of invasive arthroscopic surgery in the treatment of tibial posterior plateau fracture

目的探讨创伤关节镜下手术治疗胫骨后侧平台骨折的临床效果。方法选取2005年1月至2013年12月我院收治的110例胫骨后侧平台骨折患者作为研究对象,采用随机数字表法将所选患者分为研究组和对照组,各55例,研究组采用创伤关节镜下手术治疗,对照组采用传统的胫骨平台后内侧或后外侧入路手术治疗,随访12个月,使用HSS、Rasmussen评分标准进行评价,综合比较两组患者膝关节功能恢复优良率、骨折复位优良率。结果随访12个月,研究组膝关节功能恢复优良率和骨折复位优良率分别为94.5%、83.6%,均高于对照组的81.8%、67.3%(P<0.05)。结论创伤关节镜下手术治疗胫骨后侧平台骨折患者效果确切,手术创伤小,术后膝关节功能恢复好,复位满意,值得临床推广使用。

Objective To investigate the clinical effects of invasive arthroscopic surgery in the treatment of tibial posterior plateau fracture. Methods 110 cases of tibial posterior plateau fracture patients in our hospital were randomly divided into research group and control group,55 cases in each group. Patients in research group were treated with traumatic arthroscopic surgery,while patients in control group were treated with traditional tibial plateau side or posterolateral approach surgery. All patients were Followed- up for 12 months,the HSS and Rasmussen criteria were used to evaluate the efficacy. The good rate of knee function recovery and fractures of the two groups were compared. Results After been followed- up for 12 months,the good rate of knee function recovery and fractures of research group were respectively 94. 5% and 83. 6%,which were higher than 81. 8%and 67. 3% of control group（ P〈0. 05）. Conclusions Invasive arthroscopic surgery in the treatment of tibial posterior plateau fracture has exact effect with small surgical trauma and better recovery of knee joint function. It is worthy of clinical widely used.
