小儿疝气腹腔镜手术的临床应用价值及围手术期的舒适护理

Clinical value of pediatric laparoscopic hernia surgery and perioperative comfort care

目的探讨我院新开展的小儿疝气腹腔镜手术的临床应用价值及舒适护理在腹腔镜下小儿疝气手术中的应用效果。方法选择2013年10月1日至2014年12月31日本院收治的1-12岁152例小儿疝气手术病例。将其中84例行腹腔镜下疝气手术患儿的手术时间与68例行普通疝气手术患儿的手术时间进行比较,然后将84例行腹腔镜下疝气手术患儿随机分成干预组42例和对照组42例,干预组应用舒适护理,对照组应用常规护理,通过评分表对两组患儿的舒适度和满意度进行比较。结果腹腔镜组手术时间明显较普通组缩短（P〈0.05）。对照组和干预组的患儿出院时舒适度比较差异有统计学意义（P〈0.05）,满意度比较差异有统计学意义（P〈0.05）。结论小儿腔镜手术以其手术时间短,创伤小、恢复快、腹部瘢痕隐蔽等优点被患儿家属接受。舒适护理在行腹腔镜下疝气手术患儿的运用,能有效提高患儿舒适度,消除患儿恐惧心理,提高手术室服务质量。

Objective To investigate the clinical value of carrying out the new pediatric laparoscopic hernia surgeryand the application effect of comfort care in pediatric laparoscopic hernia surgery. Methods During October 1st,2013 and December 31 st,2014,152 cases ofchildren during 1- 12 years old received hernia surgery in our hospital were chose. Operative time of 84 children underwent laparoscopic hernia surgery and 68 children received ordinary routine hernia surgery were compared,and then the 84 children underwent laparoscopic hernia surgery were randomly divided into intervention group and control group,42 cases in each group. Patients in the intervention group received comfort care,meanwhile,the control group received usual care,thecomfort and satisfaction of children were compared. Results laparoscopic operative time was significantly shorter than normal group（ P〈0. 05）. Difference of comfort in the intervention group and the control group was statistically significant（ P〈0. 05）,as well as the satisfaction between the two groups（ P〈0. 05）. Conclusions Pediatric laparoscopic surgery withadvantages such as shorter operative time,less trauma,quicker recovery and abdominal scar hidden,and iswidely accepted by parents. The use of laparoscopichernia surgery can effectively improve the children＇s comfort,eliminating the fear of children and improve the quality of service in the operating room.
