复方生地合剂联合西药治疗系统性红斑狼疮阴虚内热型33例临床观察

Clinical Observation on 33 Cases of Systemic Lumpus Erythematosus with Syndrome of Internal Heat and Yin Deficiency Treated with Fufang Shengdi Heji(复方生地合剂) Combined with Western Medicine

目的评价复方生地合剂联合西药治疗系统性红斑狼疮（SLE）阴虚内热型的临床疗效。方法将66例SLE阴虚内热型患者随机分为治疗组与对照组各33例,两组均给予醋酸泼尼松片0.2~1 mg/（kg·d）,硫酸羟基氯喹片每次0.1 g,每日2次,口服;治疗组在此基础上给予复方生地合剂每次30 ml,每日3次口服,两组均治疗3个月。比较两组患者临床疗效,并观察治疗前后两组患者系统性红斑狼疮疾病活动指数（SLE-DAI）、中医证候积分、相关免疫指标[免疫球蛋白G（Ig G）、补体C3、补体C4与抗双链DNA（抗ds-DNA）]、24 h尿蛋白定量及激素日用量。结果治疗组临床疗效总有效率83.33%,对照组为60.00%,两组比较差异有统计学意义（P〈0.05）。治疗后两组中医证候积分、SLE-DAI、Ig G及抗ds-DNA、激素日用量及治疗组24 h尿蛋白定量均较治疗前明显降低,而补体C3升高（P〈0.05或P〈0.01）;两组治疗后比较,治疗组Ig G、激素日用量均低于对照组（P〈0.05）。结论复方生地合剂联合西药治疗SLE阴虚内热型疗效确切,且能减少患者激素日用量。

Objective To evaluate clinical effects of Fufang Shengdi Heji（ 复 方 生 地 合 剂） combined with western medicine in the treatment of 33 cases of systemic lumpus erythematosus with syndrome of internal heat and yin deficiency. Methods Totally 66 cases of SLE with syndrome of internal heat due to yin deficiency were randomly divided into a treatment group and a control group,with 33 cases in each. Both groups were given orally with 0. 2 ~1 mg /（ kg· d） of prednisone acetate tablets and 0. 1 g of hydroxychloroquine sulfate tablets each time,twice per day. The treatment group was added with Fufang Shengdi Heji（ 复方生地合剂）,30 ml each time,three times per day. Both groups were treated for 3 months. Clinical effects on patients in both groups were compared,and systemic lupus erythematosus disease activity index（ SLE-DAI）,TCM syndrome scoring,related immune indexes [immune globulin G（ Ig G）,complement C3,complement C4,anti-double strand DNA（ anti-ds DNA）,24-hour proteinuria,and hormone daily doses in both groups were observed before and after the treatment. Results Total effective rate of clinical effects was 83. 33% in treatment group,and 60. 00% in control group,with. significant difference between the two groups（ P〈0. 05）. TCM syndrome scoring,SLE-DAI,Ig G,anti-ds DNA and hormone daily doses in both groups,and 24-hour proteinuria in treatment group all decreased,while complement C3 increased in both groups after treatment（ P〈0. 05 or P〈0. 01）,especially Ig G and hormone daily doses in treatment group（ P〈0. 05）.Conclusion Fufang Shengdi Heji（ 复方生地合剂） combined with western medicine is effective in treating SLE with syndrome of internal heat and yin deficiency,and it can decrease patients＇ hormone daily doses.
