无创BiPAP呼吸机在阻塞性睡眠呼吸暂停综合征治疗中的应用 被引量:8

摘要 目的探讨无创双水平气道正压(Bi PAP)呼吸机治疗阻塞性睡眠呼吸暂停综合征(OSAS)的临床效果。方法将130例OSAS患者随机分为对照组和治疗组,每组各65例。对照组行常规保守治疗;治疗组则在此基础上加用无创Bi PAP呼吸机治疗。比较两组患者的生命体征、血气分析及血流动力学指标。结果治疗组的心率、呼吸频率、动脉压均明显低于对照组(P<0.01),夜间血氧饱和度和夜间最低血氧饱和度则明显高于对照组(P<0.01)。治疗组的氧分压、左室射血分数明显高于对照组(P<0.01),二氧化碳分压则明显低于对照组(P<0.01)。治疗组的甘油三酯明显低于对照组(P<0.01)。结论采用无创Bi PAP呼吸机治疗OSAS患者,可有效缓解患者的缺氧状态,改善临床症状,提高临床的治疗效果。 Objective To explore the clinical effectiveness of application of non-invasive Bi PAP(Bilevel Positive Airway) respirator in patients with OSAS(Obstructive Sleep Apnea Syndrome). MethodsAltogether 130 cases of OSAS were randomly and evenly divided into Control Group(65 cases) and Experiment Group(65 cases). Conventional conservative treatment was performed in both Control Group and Experiment Group, on the basis of which Experiment Group was treatment with the non-invasive Bi PAP respirator additionally. Then, indexes of vital signs, blood gas analysis and hemodynamics were compared between the two groups post-operatively. Results Experiment Group showed remarkably lower heart rate, breathing frequency and arterial pressure than that in Control Group(P〈0.01). But the nighttime blood oxygen saturation and the lowest nighttime blood oxygen saturation in Experiment Group were much higher than Control Group(P〈0.01). Partial pressure of oxygen and LVEF(Left Ventricular Ejection Fraction) in Experiment Group were much higher than Control Group(P〈0.01); while, partial pressure of carbon dioxide and the level of triglyceride in Experiment Group were much lower than Control Group(P〈0.01). Conclusion Application of non-invasive Bi PAP in treatment of OSAS patients could relieve the hypoxia state and improve clinical symptoms for patients, which raised the effectiveness of clinical treatment.
