胱抑素C与基质金属蛋白酶9在急性脑梗死中临床价值的探讨

Discussion on cystatin C and matrix metalloproteinase 9 in patients with acute cerebral infarction in the clinical value

目的探讨急性脑梗死患者血清胱抑素C(Cys C)与基质金属蛋白酶-9(MMP-9)的水平变化及临床价值。方法随机选取92例急性脑封死患者作为观察组,同期健康体检者85例作为对照组,分别检测两组血清Cys C及MMP-9水平。于治疗后5d及10d时复查观察组上述两项指标。对观察组行颈部血管彩超检查并分级。结果观察组治疗前Cys C及MMP-9水平均高于对照组[(1.27±0.35)mg/L,(0.86±0.22)mg/L;(183.68±58.92)μg/L,(118.46±45.83)μg/L,P<0.05]。治疗后10d观察组血清Cys C、MMP-9水平较治疗前下降[(0.95±0.21)mg/L,(1.27±0.35)mg/L;(156.37±52.84)μg/L,(118.46±45.83)μg/L,P<0.05]。颈动脉狭窄组血清Cys C及MMP-9水平均高于无狭窄组,差异有统计学意义(P<0.05)。结论血清Cys C及MMP-9水平在急性脑梗死时明显升高,且与颈动脉狭窄程度密切相关。在脑梗死的风险预判、病情评估、疗效及预后上有重要临床价值。

Objective To explore the change of serum cystain C（Cys C）and matrix metalloproteinase-9（MMP-9）concentration in acute cerebral infraction（ACI）patients and their clinical significance.Methods 92 patients with acute cerebral infraction were chosen randomly as observational group,and 85 health examination were chosen as the control group.Measuring the level of Cys C and MMP-9in the two groups separately.The observational group were grading by carotid duplex ultrasound.Results The concentration of Cys C and MMP-9in observational group were higher than control group before treatment[（1.27±0.35）mg/L,（0.86±0.22）mg/L;（183.68±58.92）μg/L,（118.46±45.83）μg/L,P〈0.05].Compared to the control group,the level of Cys C and MMP-9in observational group were decreased after treatment[（0.95±0.21）mg/L,（1.27±0.35）mg/L;（156.37±52.84）μg/L,（118.46±45.83）μg/L,P〈0.05].The concentration of Cys C and MMP-9in carotid stenosis group is much higher（P〈0.05）.Conclusion In the patients of ACI,Cys C and MMP-9were improved significantly,and the degree of Carotid artery stenosis had contact with them closely.The level of Cys C and MMP-9therefore might have great value in the clinical significance of ACI.
