尼莫地平对不同时间点高血压脑出血患者血清炎性因子影响的实验研究 被引量:34

摘要 目的观察尼莫地平对不同时间点高血压脑出血患者血清炎性因子的影响。方法实验分3组,选取2012年4月至2014年2月我院收治的高血压脑出血患者80例,随机分为:常规治疗组采用常规治疗(n=40例),尼莫地平治疗组在常规治疗的基础上加尼莫地平治疗(n=40例),2组疗程均4周;同期住院的中枢神经系统非器质性疾病患者20例作为对照组;于24h、48h、72h、7d、14d、28d抽取患者肘静脉血,采用酶联免疫法检测各组血清中炎性因子IL-4、IL-6和IL-10蛋白含量的变化。结果常规治疗组患者血清炎性因子IL-4、IL-6和IL-10蛋白含量在脑出血后24h、48h内开始上升,至72h达高峰,而7d、14d时逐渐下降,但仍高于对照组,两组间差异均有统计学意义(P<0.01);至28d时基本接近正常,此时与对照组比较差异无统计学意义(P>0.05);尼莫地平治疗组患者血清炎性因子IL-4、IL-6和IL-10蛋白含量在脑出血后24h、48h内开始上升,至72h达高峰,而7d时逐渐下降,但仍高于对照组,两组间差异均有统计学意义(P<0.05);至14d、28d时基本接近正常,与对照组比较差异无统计学意义(P>0.05)。尼莫地平治疗组的IL-4、IL-6和IL-10蛋白含量在24h、48h、72h、7d、14d均低于常规治疗组,差异有统计学意义(P<0.01),28d时二者之间差异无统计学意义(P>0.05)。结论通过动态监测患者血清炎性因子的变化,提示尼莫地平能较早下调患者IL-4、IL-6和IL-10的蛋白含量,抑制炎症反应,缩短病变进程保护脑组织,从而提高临床疗效。 Objective To observe the effect of nimodipine on the blood serum inflammatory factors in patients with hypertensive brain at different time points.Methods The experiment was divided into 3groups,80 patients from 2012 April to 2014 February in our hospital were hypertensive cerebral hemorrhage,were randomly divided into:normal treatment group with conventional therapy(n=40),nimodipine treatment group and nimodipine on the basis of routine treatment(n=40cases),2groups were treated for 4weeks;Patients with non-organic disease as a control group of 20 cases of central nervous system during the same hospitalization period;in 24 h,48h,72 h,7d,14 d,28dwere collected venous blood,using enzyme-linked immunosorbent assay for the detection of changes of serum inflammatory factors IL-4,IL-6and IL-10 protein content.Results The conventional treatment group serum inflammatory factors in patients with IL-4,IL-6and IL-10 protein content in the cerebral hemorrhage after 24 h,48hbegan to increase,reached the peak at 72 h,7d,14 dgradually decreased,but still higher than that of the control gro-up,there were significant differences between two groups(P〈0.01);At 28 dof the basic close to normal,compared with the control group,the difference was not statistically significant(P〉0.05);nimodipine treatment group serum inflammatory factors in patients with IL-4,IL-6and IL-10 protein content in the cerebral hemorrhage after 24 h,48hbegan to increase,reached the peak at 72 h,and 7ddecreased,but still higher than that of the control group,there were significant differences between two groups(P〈0.05);and 14 d,28dwere close to normal,compared with the control group,the difference was not statistically significant(P〉0.05).Nimodipine treatment group IL-4,IL-6and IL-10 protein levels were lower than the conventional treatment group at 24 h,48h,72 h,7d,14 d,the difference was statistically significant(P〈0.01),there was no significant difference between the 28dtwo(P〈0.05).Conclusion Through monitoring the dynamic changes in serum inflammatory factors in patients with protein content,suggesting that nimodipine can early downregulation of IL-4,IL-6and IL-10 patients,inhibit the inflammatory reaction of brain tissue,shorten the process of protection,so as to improve the clinical efficacy.
