活血止痛膏治疗全髋关节置换术后关节疼痛38例

Huoxue Zhitong Plaster for Treating Joint Pain after Total Hip Arthroplasty in 38 Cases

目的观察活血止痛膏治疗全髋关节置换术后关节疼痛的临床疗效。方法将76例患者随机分为对照组和观察组,各38例。对照组患者采用双氯芬酸二乙胺乳胶剂治疗,观察组患者采用活血止痛膏治疗,疗程均为12周。分别观察两组患者治疗后第6,12周疼痛视觉模拟评分法(VAS)评分(包括VASd及VASn)、髋关节活动度(包括屈曲、内外旋及外展幅度)及Harris髋关节功能评分,记录治疗过程中不良反应的发生情况,评价患者疗效。结果观察组患者总有效率为92.11%,稍高于对照组的89.47%,但差异无统计学意义(P>0.05);两组患者第12周VAS评分(包括VASd及VASn)较第6周均明显降低(P<0.05),观察组患者VASn评分较对照组下降更明显(P<0.05);两组患者治疗12周髋关节活动度及Harris评分均显著高于治疗第6周,但组间比较差异无统计学意义(P>0.05);两组患者不良反应发生情况比较,差异无统计学意义(P>0.05)。结论活血止痛膏治疗全髋关节置换术后关节疼痛疗效确切,过程安全,值得临床推广。

Objective To observe the clinical curative effect of Huoxue Analgesic Plaster in treating of joint pain after total hip arthroplasty. Methods Totally 76 patients were randomly divided into the control group and the observation group,38 cases in each group. The control group was treated with Diclofenac Diethylamine Emulsion,while the observation group adopted Huoxue Zhitong Plaster. The treatment of both groups asted for 12 weeks. The Visual Analogue Scale（ VAS,including VASd and VASn） scores,activity of hip joint（ including flexion,internal and external rotation and abduction ranges） and Harris hip function scores were observed at 6,12 weeks after treatment in the two groups. The occurrence situation of adverse reactions during treatment was recorded. At the same time,the therapeutic effects were evaluated. Results The total effective rate in the observation group was 92. 11%, which was higher than89. 47% in the control group,but there was no significant difference between the two groups（ P〉0. 05）. The VAS scores at 12 weeks（ including VASd and VASn） in the two groups were significantly decreased compared with those at 6 weeks（ P〈0. 05）,moreover the VASn score decrease in the observation group was more significant than that in the control（ P〈0. 05）. The activity of hip joint and Harris scores at 12 weeks in the two groups were significantly higher than those at 6 weeks,but the difference between the two groups had no statistical significance（ P〉0. 05）. The occurrence situation of adverse reaction showed no statistically significant difference between the two groups（ P〉0. 05）. Conclusion Huoxue Zhitong Plaster in treating joint pain after total hip arthroplasty is definitely effective and safe during the treatment course and deserves to be promoted.
