^(18)F-FDG PET/CT评估乳腺癌术后复发、转移及预后的价值 被引量:1

Value of ^(18)F-FDG PET/CT in evaluating recurrence,metastasis and prognosis of patients with breast cancer after surgery

摘要 目的探讨18F-氟代脱氧葡萄糖(18F-FDG)PET/CT评价乳腺癌术后复发、转移及预后的应用价值。方法回顾性分析43例乳腺癌术后患者相关临床及影像检查资料,比较18 F-FDG PET/CT检查标准摄取最大值(SUVmax)变化,采用Spearman相关性分析病灶SUVmax与预后的关系。结果 18F-FDG PET/CT对术后乳腺癌复发或转移灶检出的灵敏度、特异度和准确度分别为96.5%、92.9%和95.3%。术后证实有乳腺癌复发或转移灶者28例。治疗后复查PET/CT结果:9例有效稳定,SUVmax较治疗前下降(4.1±2.8vs.8.3±4.9)(P<0.05);7例病情恶化,SUVmax较治疗前上升(7.9±3.8vs.4.3±2.3)(P<0.05);带病生存18例的SUVmax低于10例进展死亡患者(5.4±2.6vs.9.4±3.2)(P<0.01)。病灶SUVmax与预后呈负相关(r=-0.55,P<0.05)。结论 18F-FDG PET/CT评估乳腺癌术后患者复发、转移、预后有重要临床价值。 Objective To explore the value of 18F-fluoro deoxidization glucose(18F-FDG)PET/CT in evaluating recurrence,metastasis and prognosis of patients with breast cancer after surgery.Methods Data of 43 patients with breast cancer after surgery were retrospectively reviewed.The changes in the maximum standardized uptake value(SUVmax)of 18F-FDG PET/CT were compared and the relationship between SUVmax and prognosis was analyzed with Spearman analysis.Results The sensitivity,specificity and accuracy of 18F-FDG PET/CT in detecting the recurrence and metastasis of breast cancer were 96.5%,92.9% and 95.3%.Breast cancer was confirmed to have reoccurrence or metastasis in 28 cases.18 F-FDG PET/CT reexaminations showed that SUVmax in 9cases with stable and effective condition after treatment was lower after treatment than that before(4.1±2.8vs.8.3±4.9)(P〈0.05),which in 7cases with deterioration was higher after treatment than that before(7.9±3.8vs.4.3±2.3)(P〈0.05).SUVmax was lower in 18 cases survival with disease than in 10 cases died from breast cancer after surgery(5.4±2.6vs.9.4±3.2)(P〈0.01).SUVmax was negatively correlated to prognosis(r=-0.55,P〈0.05).Conclusion 18F-FDG PET/CT has an important clinical value in evaluating the recurrence,metastasis and prognosis of the patients with breast cancer after surgery.
