低位直肠癌保肛手术的治疗效果评价

目的评价低位直肠癌保肛手术的临床治疗效果,分析其安全性和可行性。方法选取2014年1月~2015年1月期间在我院进行低位直肠癌保肛手术的67例患者进行本次研究,根据治疗方式的不同将其分为对照组（n=30）和腹腔镜（n=37）组,对照组应用传统的开腹直肠癌全系膜切除手术治疗,腹腔镜组在腹腔镜下行直肠癌全系膜切除手术治疗,观察治疗效果。结果腹腔镜组平均手术时间、术中出血量、每天的排便次数、手术后的控制大便时间等项指标均优于对照组,P〈0.05。随访显示,腹腔镜组患者的局部复发率和5年内的生存率差以及并发症发生率比较,差异无统计学意义,P〉0.05。结论在腹腔镜下行低位直肠癌保肛手术能缩短患者的手术时间、减少每天的排便次数,恢复控制排便时间短,有效减少并发症的发生率,安全、可靠、效果明显。

Objective To evaluacate of low sphincter preserving surgery colorectal cancer clinical treatment,its safety and feasibility analysis.Methods 67 patients were select from January 2014 to January 2015 in our hospital were lower sphincter preserving surgery of colorectal cancer in this study were based on different treatment modalities will be divided into a control group and the laparoscopic group and 30 patients in the control group colorectal cancer patients with traditional open total mesorectal excision surgery,the laparoscopic group,37 patients underwent laparoscopic colorectal cancer total mesorectal excision surgery,two groups were observed in patients undergoing treatment.Results Laparoscopic mean operative time,blood loss,number of bowel movements per day,time and other indexes to control bowel movements after surgery than the control group,p0.05.After 1-5 years of follow-up,the local recurrence rate in patients with laparoscopic group and poor survival and complication rate in five years,the difference was not statistically significant,p0.05.Conclusion In laparoscopic surgery can lower colorectal cancer sphincter preserving surgery in patients shorten the time and reduce the number of bowel movements per day,a short time to restore bowel control,effectively reducing the incidence of complications,safe and reliable,the effect is obvious.
