摘要
目的分析黄葵胶囊对糖尿病肾病Ⅲ-Ⅳ期患者血肌酐(Scr)、24 h尿蛋白、血清胱抑素-C(Cys-C)、血清C反应蛋白(CRP)、肿瘤坏死因子-α(TNF-α)、白介素-1β(IL-1β)水平的影响,探讨其对糖尿病肾病的治疗效果。方法 110例糖尿病肾病Ⅲ-Ⅳ期患者,以随机数字表法分为观察组与对照组,各55例。对照组给予常规治疗,观察组在对照组基础上加用黄葵胶囊治疗。比较两组患者治疗前、后Scr、24 h尿蛋白、Cys-C、CRP、TNF-α,IL-1β水平的差异。结果治疗后,观察组与对照组比较,Scr[(98.23±12.81)μmol/L VS(132.87±16.49)μmol/L,t=3.036]、24 h尿蛋白[(0.78±0.23)g/24 h VS(1.14±0.32)g/24 h,t=2.693]、Cys-C[(1.03±0.26)mg/L VS(1.59±0.24)mg/L,t=2.975]、CRP[(11.27±2.84)mg/L VS(19.37±3.02)mg/L,t=3.025]、IL-1β[(14.91±2.05)pg/ml VS(19.67±2.31)pg/ml,t=2.903]、TNF-α[(23.69±5.02)pg/ml VS(31.45±6.82)pg/ml,t=3.047]水平明显下降,两组比较差异有统计学意义(P〈0.05)。结论黄葵胶囊能明显减少糖尿病肾病Ⅲ-Ⅳ期患者尿蛋白,降低Cys-C水平,延缓Scr水平上升,起到保护肾脏功能的作用。分析其作用机制可能与黄葵胶囊调节血清微炎性反应作用有关。
Objective To analyze influence of Huangkui capsule on serum creatinine(Scr), 24 h urine protein, serum cystatin-C(Cys-C), serum C-reactive protein(CRP), tumor necrosis factor-α(TNF-α), and interleukin-1β(IL-1β) in stage Ⅲ-Ⅳ diabetic nephropathy patients, and to investigate its curative effect for diabetic nephropathy. Methods A total of 110 stage Ⅲ-Ⅳ diabetic nephropathy patients were divided by random number table into observation group and control group, with 55 cases in each group. The control group received conventional treatment, and the observation group received additional Huangkui capsule for treatment. Comparisons were made on changes of Scr, 24 h urine protein, Cys-C, CRP, TNF-α, and IL-1β levels before and after treatment in the two groups. Results After treatment comparing with the control group, the observation group had obviously decreased Scr [(98.23±12.81)μmol/L VS(132.87±16.49)μmol/L, t=3.036], 24 h urine protein [(0.78±0.23)g/24 h VS(1.14±0.32)g/24 h, t=2.693], Cys-C [(1.03±0.26)mg/L VS(1.59±0.24)mg/L, t=2.975], CRP [(11.27±2.84)mg/L VS(19.37±3.02)mg/L, t=3.025], IL-1β [(14.91±2.05)pg/ml VS(19.67±2.31)pg/ml, t=2.903], and TNF-α [(23.69±5.02)pg/ml VS(31.45±6.82)pg/ml, t=3.047] levels. The differences between the two groups had statistical significance(P〈0.05). Conclusion Huangkui capsule can remarkably reduce urine protein, decrease Cys-C level, and suspend increasing Scr level in stage Ⅲ -Ⅳ diabetic nephropathy patients. It provides protective effect for renal function, and its mechanism may have relationship with its regulating effect for serum microinflammation reaction.
