手法整复结合颈部肌肉锻炼治疗寰枢关节紊乱的临床效果分析

Clinical analysis effect of manipulative reduction combined with cervical muscle exercise in treatment of atlantoaxial joint disorder

目的分析手法整复结合颈部肌肉锻炼治疗寰枢关节紊乱的临床疗效。方法选取本院2009年1月~2013年12月收治的208例寰枢关节紊乱综合征患者为研究对象,入院后按照抽签随机取样法将其分为对照组（n=104）和观察组（n=104）。对照组给予手法整复治疗,观察组在对照组基础上给予颈部肌肉锻炼。比较两组患者治疗前后寰齿前隙,颈椎曲度深度、指数、角度,临床症状评分变化。比较两组患者总有效率。结果治疗前,两组患者各临床指标比较,差异无统计学意义（P〉0.05）,治疗后,两组患者各临床指标均明显改善,观察组寰齿前隙、临床症状评分显著低于对照组（P〈0.05）,其余指标均显著高于对照组（P〈0.05）,观察组总有效率显著高于对照组（96.15%vs 79.81%,χ2=13.139,P=0.000）。结论手法整复结合颈部肌肉锻炼治疗寰枢关节紊乱综合征患者,能显著改善其临床症状,疗效确切,值得临床推广应用。

Objective To analyze the clinical curative effect of manipulative reduction combined with cervical muscle exercise in treatment of atlantoaxial joint disorder. Methods 208 cases of patients with atlantoaxial joint disorder syndrome treated in our hospital from January 2009 to December 2013 were selected as the research object and were randomly divided into the control group（rt=104） and observation group（n=104） after admission.The patients in control group were given the treatment of manipulative reduction while the patients in observation group were given the treatment of neck muscle exercise on the basis of the treatment in control group.The Changes of atlanto-odontoid interval,cervical curvature index,depth,angle and clinical symptom scores of the patients between two groups was compared before and after the treatment respectively.The total efficiency between the two groups was compared. Results Before the treatment,the clinical indexes of patients between two groups were not statistically significant （P〉0.05）.But after the treatment,the clinical indexes between two groups of patients was significantly improved respectively.The atlanto-edontoid interval and clinical symptom scores in observation group was significantly lower than that of control group respectively （P〈0.05）,and other indexes was significantly higher than that in control group respectively （P〈0.05）.The total effective rate in observation group was significantly higher than that of control group （96.15% vs 79.81%,χ2=13.139,P=0.000）. Conclusion For the patients with atlantoaxial joint disorder syndrome,the treatment of manipulative reduction combined with cervical muscle exercise can significantly improve the clinical symptoms and the curative effect is very goed,which is worthy of clinical application.
